超声对颈动脉蹼的诊断价值

Diagnostic value of ultrasound for carotid web

目的:探讨超声评估颈动脉蹼的价值。方法:从2015年10月至2019年11月在沧州市人民医院因脑卒中或短暂性脑缺血发作入院进行头颈血管检查的患者中抽取经CT血管造影(CTA)诊断明确的颈动脉蹼患者16例,均行颈动脉超声检查。观察并记录颈动脉蹼的声像图特点,测量颈动脉蹼的长度及狭窄程度并与CTA结果对照。结果:16例颈动脉蹼患者中,超声诊断出11例,诊断符合率为68.8%。超声显示颈动脉蹼长度为2.3~4.6 mm,平均3.5 mm;CTA显示颈动脉蹼长度为2.2~4.8 mm,平均3.4 mm,差异未见统计学意义(P>0.05)。CTA诊断16例颈动脉蹼均为轻度狭窄,超声诊断出的11例颈动脉蹼亦均为轻度狭窄,符合率为100.0%。11例颈动脉蹼患者中8例为等回声,3例为高回声,彩色多普勒超声显示颈动脉蹼处的血流缓慢或回流,1例血栓形成。结论:超声诊断颈动脉蹼有一定的价值,可用于检测血栓。

Objective To assess the value of ultrasound in diagnosis of carotid web.Methods Sixteen patients who were admitted to Cangzhou People’s Hospital from October 2015 to November 2019 and underwent head and neck vascular examination for stroke or transient ischemic attack were selected.And they were diagnosed as carotid web by computed tomography angiography(CTA).All patients underwent ultrasound.The characteristics of ultrasonography of carotid web were observed and recorded.The length and stenosis of carotid web were measured and compared with the results of CTA.Results Among 16 cases of carotid web,11 cases were diagnosed by ultrasound,the diagnostic accuracy rate was 68.8%.Ultrasound showed that the length of carotid web was 2.3-4.6 mm,the average length was 3.5 mm;CTA showed that the length of carotid web was 2.2-4.8 mm,the average length was 3.4 mm,and the difference was not statistically significant(P>0.05).CTA showed mild stenosis in 16 cases,and ultrasound showed mild stenosis in 11 cases,and the coincidence rate was 100%.Eight of 11 cases were isoechoic,3 cases were hyperechoic,1 case had thrombus,and color Doppler showed that the blood flow at the carotid web was slow or reflow.Conclusions Ultrasonography has certain value in the diagnosis of carotid web,and can be used to detect thrombus.
