儿童卵黄管病变的临床特征分析

Clinical characteristics of vitelline duct lesions in children

目的探讨儿童期卵黄管病变的临床特点。方法抽取2017年8月至2019年8月于西安市儿童医院经手术病理确诊的124例卵黄管病变患儿为研究对象。采用回顾性分析方法,分析其性别、年龄、并发症等一般临床资料,术后病理类型及异位黏膜发生情况。结果124例卵黄管病变患儿中,男性多于女性,男∶女为2.3∶1。Meckel憩室在卵黄管病变中最为常见,且好发于男性,男∶女为3.9∶1,消化道出血是其最常见的并发症。胃黏膜是卵黄管病变中最常见异位组织。异位组织的残留与性别有相关性(χ^2=10.9218,P<0.05),其在男性患儿中发病率较高。并发症的出现与异位组织残留有着密切关系(χ^2=52.0451,P<0.01),其中消化道出血者组织异位率最高。结论儿童卵黄管病变的临床表现多样,掌握其不同类型的临床特征,对快速诊断及早期治疗该病十分重要。

Objective To investigate the clinical features of vitelline duct lesions in children.Methods A total of 124 children with vitelline duct lesions diagnosed by operation and pathology in Xi’an Children’s Hospital from August 2017 to August 2019 were selected for the study.The general clinical data(such as gender,age,complications),postoperative pathological types and occurrence of ectopic mucosa were analyzed by retrospective analysis.Results Among the 124 children with vitelline duct lesions,there were more males than females,and the ratio of male to female was 2.3∶1.Meckel diverticulum was the most common type of vitelline duct lesions,and males were prone to developing the disease,with a male-to-female ratio of 3.9∶1.Gastrointestinal bleeding was the most common complication.Gastric mucosa was the most common ectopic tissue in vitelline duct lesions.There was a significant correlation between residual ectopic tissue and gender(χ^2=10.9218,P<0.05),of which the incidence was higher in male children.The occurrence of complications was closely related to the residue of ectopic tissue(χ^2=52.0451,P<0.01),and the rate of ectopic tissue was the highest in children with gastrointestinal bleeding.Conclusions The clinical manifestations of vitelline duct disease in children are various.Mastering different clinic features is very important for rapid accurate diagnosis,early treatment of the disease.
