动态心电图联合心脏彩超对老年患者阵发性心房颤动的诊断价值

Value of dynamic electrocardiogram combined with color Doppler echocardiography in the diagnosis of paroxysmal atrial fibrillation in the elderly patients

目的探讨动态心电图联合心脏彩超对老年患者阵发性心房颤动的诊断价值。方法抽取2016年1月至2020年1月在平煤神马医疗集团总医院诊治的疑似阵发性心房颤动老年患者90例。所有患者均分别进行动态心电图、心脏彩超检查,以临床综合诊断结果为金标准,分析动态心电图联合心脏彩超诊断老年阵发性心房颤动的敏感性、特异性及准确性。结果动态心电图对老年患者阵发性心房颤动的敏感性、特异性、准确性分别为97.53%、88.89%、96.67%;心脏彩超对老年患者阵发性心房颤动的敏感性、特异性、准确性分别为91.36%、66.67%、88.89%;联合检测对老年患者阵发性心房颤动的敏感性、特异性、准确性分别98.77%、100.00%、98.89%;虽然联合检测的敏感性、特异性及准确性均高于其他两种方法,但三种方法比较差异未见统计学意义(P>0.05)。结论动态心电图与心脏彩超在诊断老年患者阵发性心房颤动方面均具有良好的诊断效能,联合使用诊断效果更佳。

Objective To investigate the diagnostic value of dynamic electrocardiogram(DCG)combined with color Doppler echocardiography for elderly patients with paroxysmal atrial fibrillation.Methods A total of 90 elderly patients with suspected paroxysmal atrial fibrillation who were diagnosed and treated in Pingmei Shenma Medical Group General Hospital from January 2016 to January 2020 were selected.All the patients were examined by DCG and color Doppler echocardiography.The sensitivity,specificity and accuracy of the diagnosis of paroxysmal atrial fibrillation in the elderly by DCG combined with color Doppler echocardiography were analyzed with the clinical comprehensive diagnosis results as gold standard.Results The sensitivity,specificity and accuracy of DCG in the diagnosis of paroxysmal atrial fibrillation in the elderly patients were 97.53%,88.89%and 96.67%respectively;the sensitivity,specificity and accuracy of color Doppler echocardiography were 91.36%,66.67%and 88.89%respectively;the sensitivity,specificity and accuracy of combined detection were 98.77%,100.00%and 98.89%respectively.The sensitivity,specificity and accuracy of the combined detection were higher than those of the other two methods,however,there was no significant difference in these indexes among the three methods(P>0.05).Conclusions DCG and color Doppler echocarcliongraphy have good diagnostic efficacy for paroxysmal atrial fibrillation in the elderly patients.And the combination of the two method has better diagnostic efficacy.
