超声对甲状腺微小乳头状癌与微小结节性甲状腺肿的鉴别诊断价值

Differential diagnosis of papillary thyroid microcarcinoma and micronodular goiter by ultrasound

目的探讨超声在甲状腺微小乳头状癌(PTMC)与微小结节性甲状腺肿(MNG)鉴别诊断中的价值。方法将2017年4月至2020年4月郑州大学附属洛阳中心医院收治的133例行超声及病理检查的甲状腺微小结节患者纳为研究对象,病理检查证实其中PTMC 46例(共52枚),MNG 87例(共91枚),观察患者超声检查图像特征,分析PTMC与MNG的超声表现。结果 PTMC及MNG患者年龄、性别差异均未见统计学意义(P均>0.05);超声检查提示,PTMC及MNG在结节数目、形态、边界、回声强度及均匀度、囊变、微小钙化、血流信号及淋巴结肿大中差异均有统计学意义(P<0.05),两者大小、声晕比较差异未见统计学意义(P>0.05)。结论超声检查中,PTMC及MNG在结节数目、形态、边界、回声强度及均匀度、囊变、微小钙化、血流信号及淋巴结肿大中中差异均有统计学意义,以上特征在超声鉴别PTMC及MNG中具有重要意义。

Objective To investigate the differential value of papillary thyroid microcarcinoma(PTMC)and micronodular goiter(MNG)by ultrasound.Methods A total of 133 patients who underwent ultrasound and pathological examination for thyroid micronodules in Luoyang Central Hospital Affiliated to Zhengzhou University from April 2017 to April 2020 were included.Pathological examination showed that there were 46 cases of PTMC(52 nodules)and 87 cases of MNG(91 nodules).The characteristics of ultrasound images were observed,and the manifestations of PTMC and MNG were summarized.Results There was no significant difference in age and gender of PTMC and MNG patients(P>0.05).Ultrasound examination indicated that PTMC and MNG had significant differences in the number,shape,boundary,echo intensity and evenness of nodules,cystic degeneration,microcalcification,blood flow signal and lymph node enlargement(P<0.05),and there was no significant difference in the size and acoustic lobe between them(P>0.05).Conclusions In ultrasonic examination,PTMC and MNG show significant differences in the number,shape,boundary,echo intensity and evenness of nodules,cystic degeneration,microcalcification,blood flow signal and lymph node enlargement.The above characteristics are of great significance in the ultrasonic identification of PTMC and MNG.
