盐酸氨溴索联合抗生素治疗新生儿感染性肺炎的疗效及安全性分析 被引量:11

Therapeutic efficacy and safety of ambroxol hydrochloride combined with antibiotics for neonatal infectious pneumonia

摘要 目的研究盐酸氨溴索联合抗生素治疗新生儿感染性肺炎(neonatal infectious pneumonia,NIP)的临床疗效及安全性。方法研究对象选取某院2015年1月~2017年1月间收治的NIP患儿96例,采用随机数字法将其分为对照组和观察组,每组48例。对照组患儿均接受常规抗生素治疗,在此基础上,观察组患儿加用盐酸氨溴索治疗。比较两组患儿的治疗总有效率和不良反应发生率、治疗后的症状体征消失时间及住院时间,同时比较两组患儿治疗前后的血气分析指标变化。结果观察组的治疗总有效率(93.75%)明显高于对照组(68.75%)(P<0.01);观察组患儿的体温恢复时间、咳喘消失时间、肺部啰音消除时间、肺部X线吸收影像、平均住院时间均明显短于对照组,每日吸痰次数明显少于对照组,差异均有统计学意义(P<0.05);观察组患儿治疗后的PaO2、TcSaO2明显高于对照组、治疗前(P<0.05),观察组患儿治疗后的PaCO2明显低于对照组、治疗前(P<0.05);两组患儿治疗期间均未发生与治疗相关的不良反应或并发症。结论盐酸氨溴索联合抗生素治疗NIP的临床疗效较好,患儿治疗后的症状体征消失时间明显短于单用抗生素治疗效果,加用盐酸氨溴索可明显改善患儿的血氧分压、血二氧化痰分压及血氧饱和度等血气分析指标,同时盐酸氨溴索的安全性较高,值得在临床推广。 Objective To study the clinical efficacy and safety of ambroxol hydrochloride combined with antibiotics in the treatment of neonatal infectious pneumonia(NIP).Methods Totally 96 newborns with NIP admitted to our hospital from January 2015 to January 2017 were randomly selected,and they were divided into control group and observation group by random number method,with 48 cases in each group.Patients in the control group were treated with conventional antibiotics,while those in the observation group were treated with ambroxol hydrochloride combined with antibiotics.The data of two groups were compared,including the total treatment efficiency,the incidence of adverse reactions,the symptom reduction and hospitalization time after treatment,and the parameter changes of blood gas analysis before and after treatment.Results The total treatment efficiency of the observation group(93.75%)was significantly higher than that of the control group(68.75%)(P<0.01).Compared with the control group,the observation group had faster temperature recovering,shorter time of cough and asthma disappearance,lung rale sound elimination,X-ray absorption images and average hospitalization,and fewer times of sputum suction.There were statistically significant differences(P<0.05).PaO2 and TcSaO2 in the observation group after the treatment were significantly higher than those in control group,and they were also higher than before(P<0.05).PaCO2 in the observation group after the treatment was significantly lower than that in the control group,and it was also higher than before(P<0.05).No treatment-related adverse events or complications were found in two groups during the period of treatment.Conclusion The clinical efficacy of ambroxol hydrochloride combined with antibiotics in the treatment of NIP is better.Patients treated with ambroxol hydrochloride combined with antibiotics have significantly shorter time of symptom reduction than those treated with antibiotics alone.Adding ambroxol hydrochloride can significantly improve PaO2,PaCO2,TcSaO2 and other indicators of blood gas analysis.And the safety of ambroxol hydrochloride is high,which is worthy of clinical promotion.
