踝关节牵开成形联合富血小板血浆注射治疗创伤性踝关节炎的临床效果

Clinical effect of ankle distraction arthroplasty combined with platelet-rich plasma injection in the treatment of traumatic ankle arthritis

目的分析踝关节牵开成形联合富血小板血浆(PRP)注射治疗创伤性踝关节炎的临床效果。方法选取我院2016年11月至2019年11月收治的118例创伤性踝关节炎患者作为研究对象,根据治疗方法的不同将其分为手术组和手术+PRP注射组,各59例。手术组采用踝关节牵开成形术治疗,手术+PRP注射组采用踝关节牵开成形联合PRP注射治疗。比较两组的临床效果。结果治疗前,两组的AAOS评分、踝关节背伸活动度、跖屈活动度比较,差异无统计学意义(P>0.05);治疗6个月后,两组的AAOS评分升高,踝关节背伸活动度、跖屈活动度均增大,且手术+PRP注射组优于手术组,差异具有统计学意义(P<0.05)。治疗6个月后,两组的BBS评分升高,SF-MPQ评分降低,且手术+PRP注射组优于手术组,差异具有统计学意义(P<0.05)。治疗6个月后,手术+PRP注射组的治疗总有效率高于手术组,差异具有统计学意义(P<0.05)。结论踝关节牵开成形联合PRP注射治疗创伤性踝关节炎患者的效果显著,可显著改善患者病情,值得临床推广应用。

Objective To analyze the clinical effect of ankle distraction arthroplasty combined with platelet-rich plasma(PRP)injection in the treatment of traumatic ankle arthritis.Methods A total of 118 patients with traumatic ankle arthritis admitted in our hospital from November 2016 to November 2019 were selected as the research objects,and the patients were divided into operation group and operation+PRP injection group according to the different treatment methods,with 59 cases in each group.The operation group was treated with ankle distraction arthroplasty,and the operation+PRP injection group was treated with ankle distraction arthroplasty combined with PRP injection.The clinical effects of the two groups were compared.Results Befor treatment,there were no significant difference in AAOS score,dorsiflexion activity and plantar flexion activity of ankle between the two groups(P>0.05);after 6 months of treatment,AAOS score in the two groups increased,dorsiflexion activity and plantar flexion activity of ankle enlarged,those in the operation+PRP injection group were better than the operation group,and the differences were statistically significant(P<0.05).After 6 months of treatment,BBS score in the two groups increased,and SF-MPQ score decreased,those in the operation+PRP injection group were better than the operation group,and the differences were statistically significant(P<0.05).After 6 months of treatment,the total effective rate of treatment in the operation+PRP injection group was higher than that in the operation group,and the difference was statistically significant(P<0.05).Conclusion The ankle distraction arthroplasty combined with PRP injection in the treatment of patients with traumatic ankle arthritis has significant effect,it can significantly improve the patients'condition,which is worthy of clinical promotion and application.
