普通二维彩色多普勒超声、超声造影及超声弹性成像在乳腺癌诊断中的价值 被引量:6

Value of ultrasound-color doppler flow imaging, contrast-enhanced ultrasound and ultrasonic elastography in the diagnosis of breast cancer

摘要 目的探讨普通二维彩色多普勒超声(US-CDFI)、超声造影(CEUS)及超声弹性成像(UE)在乳腺癌诊断中的价值。方法选取我院2017年1月至2019年12月收治的120例疑似乳腺癌患者,对患者实施US-CDFI、CEUS、UE检查。以手术病理诊断结果为金标准。比较分析US-CDFI、CEUS、UE诊断乳腺癌的结果及诊断结果一致性,并比较乳腺癌与乳腺良性病变的超声弹性参数。结果手术病理诊断确诊阳性89例、阴性31例。CEUS、UE在乳腺癌诊断中的灵敏度、特异度、准确度、阳性预测值及阴性预测值均高于US-CDFI,差异具有统计学意义(P<0.05)。US-CDFI与手术病理诊断结果之间的一致性为中度,CEUS、UE与手术病理诊断结果之间的一致性均为高度。乳腺癌患者的应变率比值、弹性值均大于乳腺良性病变患者,差异具有统计学意义(P<0.05)。结论在乳腺癌诊断中,CEUS、UE的诊断价值高于US-CDFI,可对乳腺癌进行灵敏、准确的诊断,值得临床推广应用。 Objective To investigate the value of ultrasound-color doppler flow imaging(US-CDFI),contrast-enhanced ultrasound(CEUS)and ultrasonic elastography(UE)in the diagnosis of breast cancer.Methods A total of 120 patients of suspected breast cancer patients in our hospital from January 2017 to December 2019 were selected,and the patients were examined by US-CDFI,CEUS and UE.Taking surgical pathological diagnosis results as the gold standard.US-CDFI,CEUS,UE diagnosis results of breast cancer and the consistency of diagnosis results were compared and analyzed,and the ultrasound elastic parameters of breast cancer and benign breast lesions were compared.Results The surgical pathological diagnosis was positive in 89 cases and negative in 31 cases.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of CEUS and UE were higher than those of US-CDFI,and the differences were statistiedlly significant(P<0.05).The consistency between US-CDFI and surgical pathological diagnosis results was moderate,and the consistency between CEUS,UE and surgical pathological diagnosis results were high.The strain rate ratio and elasticity value of breast cancer patients were higher than those of breast benign lesions,and the differences were statistiedlly significant(P<0.05).Conclusion In the diagnosis of breast cancer,the diagnostic value of CEUS and UE is higher than that of US-CDFI,which can make sensitive and accurate diagnosis of breast cancer,and is worthy of clinical promotion and application.
