精益管理模式在骨科外来器械管理中的应用效果

Application effect of lean management mode in the management of external orthopedic devices

目的探讨精益管理模式[也称丰田生产系统(TPS)]在骨科外来器械管理中的应用效果。方法选择2018年7月至2018年12月实施TPS前接受外来器械治疗的患者182例次作为对照组,选择2019年1月至2019年6月实施TPS后接受外来器械治疗的患者185例次作为观察组。通过TPS找出影响骨科外来器械管理中的相关因素,制定并实施改进计划。比较两组的管理效果。结果观察组的患者信息错误、器械数量错误、器械记录错误发生率均低于对照组(P<0.05)。观察组术后器械去向规范率高于对照组,术后器械遗失率低于对照组,差异具有统计学意义(P<0.05)。医务人员对观察组的外来器械包满意度评分显著高于对照组,差异有统计学意义(P<0.05)。结论应用TPS模式可以使外来器械供应、准备与使用规范化、制度化,提高管理效率,降低医疗风险,增强CSSD工作人员的责任意识,提高了手术医护人员的满意度。

Objective To explore the application effect of lean management mode[also known as Toyota Production System(TPS)]in the management of external orthopedic devices.Methods A total of 182 cases of patients receiving external device treatment before TPS implementation from July 2018 to December 2018 were selected as control group,and 185 cases of patients receiving external device treatment after TPS implementation from January 2019 to June 2019 were selected as observation group.The related factors affecting the management of external orthopedic devices were identified through TPS,then the improvement plan was formulated and implemented.The management effects of the two groups were compared.Results The incidences of information errors of patients,number errors of devices and recording errors of devices in the observation group were lower than those in the control group(P<0.05).The standardized rate of postoperative device whereabouts in the observation group was higher than that in the control group,the lost rate of postoperative instrument was lower than that in the control group,and the differences were statistically significant(P<0.05).The satisfaction score of medical staff to the external instrument set of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion TPS mode can make the supply,preparation and use of external devices standardized and institutionalized,improve the management efficiency,reduce the medical risk,enhance the sense of responsibility of CSSD staff,and improve the satisfaction of surgical medical staff.
