基于达标理论的干预方案对肝硬化患者出院准备度的影响

Influence of Intervention Scheme Based on the Theory of Goal Attainment on Discharge Readiness of Patients with Cirrhosis

目的探讨基于达标理论的干预方案对肝硬化患者出院准备度的影响。方法方便抽样选取山西省某三级甲等医院感染科99例肝硬化患者为研究对象,按入院时间分为对照组和干预组。对照组(49例)实施常规护理,干预组(50例)实施基于达标理论的干预方案,比较两组出院准备度、自我管理行为,疾病不确定感和非计划再人院情况。结果干预组出院准备度和自我管理行为优于对照组(P<0.01),疾病不确定感得分低于对照组(P<0.01),干预组出院后1个月和3个月的非计划再入院率均低于对照组(均P<0.01)。结论基于达标理论的干预方案,有助于提高肝硬化患者出院准备度和自我管理行为,降低患者疾病确定感和非计划再入院率,可为临床出院准备工作提供参考。

Objective To explore the influence of the intervention scheme based on the theory of goal at-tainment on the discharge readiness of patients with cirrhosis.Methods By convenience sampling method,99 patients with cirrhosis from the department of infection in Shanxi Province were selected as the researchobjects and divided into control group and intervention group according to the admission time.The controlgroup(49 cases)received routine nursing care,and the intervention group(50 cases)implemented the in-tervention scheme based on the theory of goal attainment.The discharge readiness,self-management behav-ior,disease uncertainty and unplanned readmission of the two groups were compared,Results The dis-charge readiness and self-management behavior of the intervention group were better than those of thecontrol group(P<0.01);and the score of disease uncertainty was lower than that of the control group(P<0.01);the unplanned readmission rates of the intervention group at 1 month and 3 months after dis-charge were lower than those of the control group(all P<0.01).Conclusion The intervention schemebased on the theory of goal attainment can improve the discharge readiness and self-management behaviorof cirrhosis patients,reduce the patient's disease certainty and unplanned readmission rate,and provide thereference for clinical discharge preparation.
