磁性医院与非磁性医院患者结局比较的Meta分析

Outcome Comparation between Magnet and Non-Magnet Hospital:A Meta-Analysis

目的明确磁性医院在改善患者结局中的效果,为磁性理念应用提供科学依据。方法W采用计算机检索Cochrane Library,JBt循证卫生保健国际合作中心图书馆、EMBASE、PubMed,Web of Science,中国生物医学文献数据库(CBM),中国期刊全文数据库(CNKI),维普数据库(VIP)和万方数据库中探讨医院磁性对患者结局影响的研究,检索时间均为1981年1月至2019年12月,纳入比较磁性医院与非磁性医院对患者结局影响的研究,由2名评价人员按照标准独立筛选文献,提取资料,评价文献质量。用RevMan 5.3软件进行Meta分析。结果共纳入9篇文献,涉及磁性医院835个,非磁性医院3662个,涵盖患者结局指标5个。与非磁性医院比较,磁性医院在抢敦失败率,压疮发生率,脑卒中患者死亡率等方面差异均有统计学意义(均P<0.05),而在手术患者30 d死亡率,住院时间等方面差异均无统计学意义(均P>0.05)。结论―磁性医院的患者结局优于非磁性医院,但在儿童患者中效果不明显。磁性医院倡导的磁性护理管理理念对改善患者照护质量有一定积极作用,但仍需结合我国国情进一步加以探索和完善。

Objective To clarify the effect of magnet state on the patients’outcomes and thus provide theevidence for its application.Methods A systemic search in Cochrane Library,JBI evidence-based healthcare international cooperation center library,EMBASE,PubMed,Web of science,CBM,CNKI,VIP andWanfang database was applied to collect studies about the effect of magnetic status on patients’outcomeswhich published from January 198l to December 2019.The studies focused on the outcome comparation be-tween magnet and non-magnet hospital were included.Data was analyzed by RevMan 5,.3 software after lit-erature screening,data extraction,and quality appraisal according to criteria by two reviewers independent-ly.Results Totally 9 studies were recruited,including 835 magnetic hospitals,3662 non-magnetic hospi-tals,and 5 patient outcome indicators.Compared with non-magnet hospitals,magnet hospitals had statisti-cally significant differences in rescue failure rate,pressure ulcer incidence rate,the mortality rate of strokepatients(all P<0.05).There were no statistically remarkable differences in 30 days mortality rate and hos-pital stay time of surgical patients(all P>0.05).Conclusions The patients’outcomes in magnet hospitalswere better than those in non-magnet hospitals,but their effect on pediatric was not remarkable.The prac-tice environment of magnet hospitals may benefit patients’outcomes,but still needs to be further exploredaccording to our culture.
