推拿结合McKenzie疗法对腰椎间盘突出症患者血清IL-1β及IL-6水平的影响

Effect of Massage Combined with McKenzie Therapy on Serum IL-1βand IL-6 in Patients with Lumbar Disc Herniation

目的:探讨推拿结合Mc Kenzie疗法对腰椎间盘突出症患者血清IL-1β及IL-6的影响。方法:将80例腰椎间盘突出症患者随机分为治疗组40例、对照组40例,治疗组以推拿手法结合Mc Kenzie疗法治疗,对照组则单纯以推拿手法治疗,分别于就诊前、疗程结束后测定两组血清IL-1β、IL-6水平的变化情况,并记录下腰痛评分(Japanese Orthopaedic Association Scores,JOA)及简化麦吉尔疼痛问卷评分(short-form McGill pain questionnaire,SF-MPQ);其中SF-MPQ评分包括疼痛分级指数评分(pain rating index,PRI)、视觉模拟评分(visual analogue scale,VAS)、现时疼痛程度评分(present pain intensity,PPI);最终对结果进行统计分析,评定疗效,推测可能的作用机制。结果:治疗组总有效率为97.5%,高于对照组的80.0%(P<0.05);治疗后,两组JOA评分、SF-MPQ评分、IL-1β水平及IL-6水平均较治疗前改善,且治疗组改善程度优于对照组(P<0.05)。结论:推拿结合Mc Kenzie疗法可能通过调节腰椎间盘突出症患者血清IL-1β及IL-6含量,进而缓解疼痛等症状,提高临床疗效。

Objective:To investigate the effect of massage combined with McKenzie therapy on serum IL-1β and IL-6 in patients with lumbar disc herniation.Methods:Eighty patients with lumbar disc herniation were divided into treatment group(40 cases)and control group(40 cases),the treatment group was treated with massage and McKenzie therapy,while control group only treated with massage.The changes of serum IL-1β and IL-6 were measured before and after treatment,and the low back pain score(JOA)and the simplified McGill pain questionnaire(SF-MPQ)were scored(including pain grading index score PRI,visual analogue score VAS,current pain score PPI).Results statistical analysis was performed to assess efficacy and speculate the possible mechanism.Results:The total effective rate of treatment group was 97.5%,which was significantly higher than 80.0% that of control group(P<0.05).After treatment,the JOA score,SF-MPQ score,IL-1β and IL-6 level were higher than those before treatment,and the improvement were better than those of control group(P<0.05).Conclusion:Massage combined with McKenzie therapy may improve the efficacy by regulating the serum levels of IL-1β and IL-6 in patients with lumbar disc herniation,thereby alleviating pain and other symptoms.
