ACT疗法对COPD稳定期患者呼吸锻炼依从性及ESCA评分的影响

Effect of ACT Therapy on Respiratory Exercise Compliance and ESCA Score in Patients with COPD at Stable Period

目的:研究接纳与承诺(ACT)疗法应用于慢性阻塞性肺疾病(COPD)稳定期的效果。方法:选取2018年6月-2019年6月本院COPD稳定期患者89例,按照入院时间分为两组,其中2018年6-12月44例为对照组,2019年1-6月45例为观察组。对照组采用常规护理,观察组在对照组基础上采用ACT疗法。比较两组呼吸锻炼依从性、干预前后自我护理能力[自我护理能力测定表(ESCA评分)]、心理灵活性[接纳与行动问卷(AAP-Ⅱ评分)]及生活质量[圣乔治呼吸问卷(SGRQ评分)]。结果:观察组总依从性95.56%,高于对照组的79.55%(P<0.05);干预后观察组ESCA评分高于对照组(P<0.05);干预后观察组AAP-Ⅱ评分低于对照组,SGRQ评分高于对照组(P<0.05)。结论:ACT疗法应用于COPD稳定期能提高呼吸锻炼依从性,提升自我护理能力及心理灵活性,改善生活质量。

Objective:To study the effect of acceptance and commitment (ACT) therapy on chronic obstructive pulmonary disease (COPD) at stable period.Method:A total of 89 patients with COPD stable period in our hospital from June 2018 to June 2019 were selected and divided into two groups according to admission time,among which 44 patients of June to December 2018 were the control group and 45 patients of January to June 2019 were the observation group.The control group received routine care,and the observation group received ACT therapy on the basis of the control group.Compared two groups of breathing exercise adherence,nursing intervention before and after the intervention self ability[self-care ability measurement table (ESCA score)],mental flexibility[acceptance and action questionnaire (AAP-Ⅱ score)]and the quality of life[st.George’s respiratory questionnaire (SGRQ scores)].Result:The total compliance of the observation group was 95.56%,which was higher than 79.55% of the control group (P<0.05).ESCA score of the observation group was higher than that of the control group after intervention (P<0.05).After the intervention,AAP-Ⅱ score of the observation group was lower than that of the control group,SGRQ score of the observation group was higher than that of control group (P<0.05).Conclusion:The application of ACT therapy in COPD stable period can improve respiratory exercise compliance,improve self-care ability and psychological flexibility,and improve the quality of life.
