术前放疗联合开放性手术治疗骨髓瘤的近远期效果观察

The Short and Long Term Effect of Preoperative Radiotherapy combined with OpenSurgery in the Treatment of Myeloma

目的探究术前放疗联合开放性手术治疗骨髓瘤的近远期疗效。方法选择108例骨髓瘤患者为研究对象,按照治疗方式的差异将其区分为实验组(术前接受放疗,49例)和对照组(术前未接受放疗,53例),对比2组患者临床治疗效果、随访生存情况以及不良反应发生率。结果(1)实验组患者ORR和DCR均高于对照组(P<0.05)。(2)实验组患者1年、2年和3年生存率分别为46.94%、36.73%和32.65%,对照组患者1年、2年和3年生存率分别为43.30%、18.87%和15.09%,组间比较实验组2年和3年生存率高于对照组(P<0.05)。(3)实验组患者中位OS及中位PFS均高于对照组(P<0.05)。(4)实验组治疗中不良反应发生率为20.41%,对照组为15.09%,2组差异不明显(P>0.05)。结论术前放疗联合开放性手术对骨髓瘤患者具有较好的治疗效果,能够显著提高临床治疗效果,延长患者生存期,同时治疗安全性较好。

Objective To explore the short-term and long-term effects of preoperative radiotherapy combined with open surgery in the treatment of myeloma.Methods 108 patients with myeloma were selected as the research subjects,and they were divided into the experimental group(49 patients underwent radiotherapy before surgery),and the control group(53 patients who did not receive radiotherapy before surgery)according to the differences in the treatment options.Compared the clinical treatment effect,follow-up survival analysis and incidence of adverse reactions of the 2 groups of patients.Results(1)1-,2-,and 3-year survival rates of the experimental group were 46.94%,36.73%,and 32.65%,and the 1-year,2-year,and 3-year survival rates of the control group were 43.30%,18.87%,and 15.09%,respectively.The 2-and 3-year survival rates in the experimental group were higher than the control group(P<0.05);(2)The median OS and median PFS of the experimental group were higher than the control group(P<0.05);(3)The incidence rate of adverse reactions in the experimental group was 20.41%,the control group was 15.09%,the difference between the 2 groups was not obvious(P>0.05).Conclusion Preoperative radiotherapy combined with open surgery has a good therapeutic effect on patients with myeloma,can significantly improve the clinical treatment effect,prolong the survival time of patients,and the treatment is safe.
