尿流动力学检查对良性前列腺增生合并糖尿病患者的临床评估价值研究

Clinical Evaluation Value of Urodynamics Examination in Patients with Benign Prostatic Hyperplasia and Diabetes Mellitus

目的:探究良性前列腺增生(BPH)合并糖尿病(DM)患者应用尿流动力学(UDS)检查的临床评估价值。方法:选择笔者所在医院53例BPH合并DM患者(研究组)和53例单纯BPH患者(对照组)作为研究对象,收治时间为2019年1月-2020年6月。两组患者均行UDS检查及经尿道前列腺等离子剜除术(TUERP)。比较两组手术前后国际前列腺症状评分(IPSS)、最大尿流率(Qmax)和术前前列腺体积、膀胱初始尿意容量(FDV)、膀胱最大容量(MCC)、残余尿量(PVR)、最大尿流时逼尿肌压力(Pmax)及逼尿肌不稳定(DI)、膀胱出口梗阻(BOO)、膀胱顺应性(BC)升高发生率。结果:两组术前IPSS、前列腺体积比较差异无统计学意义(P>0.05);研究组FDV、MCC、PVR及术后IPSS均高于对照组,且DI、BOO及BC升高发生率明显高于对照组,而手术前后Qmax及术前Pmax均明显低于对照组,差异有统计学意义(P<0.05)。结论:BPH合并糖尿病DM患者应用UDS检查的临床评估价值显著,值得推广。

Objective:To explore the clinical evaluation value of urodynamics (UDS) examination in patients with benign prostatic hyperplasia (BPH) and diabetes mellitus (DM).Method:Fifty-three patients with BPH and DM (study group) and 53 patients with BPH alone (control group) in our hospital were selected as the research objects.The admission and treatment time was from January 2019 to June 2020.Both groups of patients underwent UDS examination and transurethral plasmakinetic enucleation of the prostate (TUERP).The international prostate symptom score (IPSS) and maximum urinary flow rate (Qmax) before and after surgery and prostate volume,bladder initial urine volume (FDV),maximum bladder capacity (MCC),residual urine volume (PVR),maximum urinary flow detrusor pressure (Pmax),and the incidence of increased of detrusor instability (DI),bladder outlet obstruction (BOO),and bladder compliance (BC) before surgery were compared between the two groups.Result:There were no significant differences in IPSS before surgery and prostate volume between the two groups (P>0.05).FDV,MCC,PVR and postoperative IPSS in the study group were all higher than those in the control group,and the incidence of increased DI,BOO and BC in the study group were significantly higher than those in the control group,while Qmax before and after surgery and Pmax before surgery were significantly lower than those in the control group,the differences were statistically significant (P<0.05).Conclusion:The clinical evaluation value of UDS in BPH and DM patients is significant,which is worthy of promotion.
