临床护理路径在阑尾炎患者腹腔镜手术后护理中的应用及对并发症发生的影响

Application of Clinical Nursing Pathway in Post-laparoscopic Nursing Care of Appendicitis Patients and Its Influence on the Occurrence of Complications

目的:探讨临床护理路径在阑尾炎患者腹腔镜手术后护理中的应用及对并发症发生的影响。方法:选择2018年11月-2020年3月于本院进行腹腔镜手术治疗的阑尾炎患者76例,根据数字随机法分为对照组和观察组各38例,对照组接受常规护理,观察组在对照组的基础上接受临床护理路径干预。比较两组患者护理后的恢复指标、并发症发生率和配合度评分。结果:护理后观察组患者的术后胃肠功能恢复时间、首次排气时间、术后抗生素使用时间、下床活动时间和住院时间均优于对照组(P<0.05);观察组护理后肠梗阻、皮下气肿、切口感染、腹腔出血、腹腔脓肿的发生率低于对照组(P<0.05);护理后观察组患者的用药配合、饮食配合、行为配合和心理配合评分均高于对照组(P<0.05)。结论:临床护理路径在阑尾炎患者腹腔镜手术后护理中效果显著,促进患者术后胃肠道恢复情况,减少术后并发症的发生,提高患者的配合度,值得推广应用。

Objective:To explore the application of clinical nursing pathways in post-laparoscopic nursing care of appendicitis patients and their influence on the occurrence of complications.Method:A total of 76 patients with appendicitis who underwent laparoscopic surgery in our hospital from November 2018 to March 2020 were selected.According to the numerical randomization method,they were divided into the control group and the observation group,38 cases in each group.The control group received routine care,and the observation group was received clinical care pathway intervention on the basis of the control group.The recovery indexes,complication incidence and cooperation scores of the two groups of patients after nursing were compared.Result:After nursing,the postoperative gastrointestinal function recovery time,first exhaust time,postoperative antibiotic usage time,time to get out of bed,and hospital stay in the observation group were better than those in the control group(P<0.05).The incidence of obstruction,subcutaneous emphysema,incision infection,abdominal hemorrhage,and abdominal abscess was lower than that of the control group(P<0.05).After nursing,the observation group’s medication,diet,behavior and psychological cooperation scores were higher than those of the control group(P<0.05).Conclusion:The clinical nursing path has a significant effect in postoperative care of patients with appendicitis,promotes the recovery of the gastrointestinal tract after surgery,reduces the occurrence of postoperative complications,and improves the cooperation of patients.It is worthy of popularization and application.
