摘要
目的:分析髂内动脉球囊阻断术在凶险性前置胎盘、胎盘植入患者剖宫产中的效果。方法:回顾性分析本院2018年10月-2020年4月凶险性前置胎盘、胎盘植入剖宫产患者共86例,根据其术中是否应用髂内动脉球囊阻断术分为对照组(n=40,行常规剖宫产术)和观察组(n=46,在对照组的基础上行髂内动脉球囊阻断术),比较两组手术时间、术中出血量、术后抗生素使用时间、子宫切除率及并发症发生率。结果:观察组手术时间及术后抗生素使用时间均短于对照组,术中出血量少于对照组(P<0.05);观察组子宫切除率、术后并发症发生率均低于对照组(P<0.05)。结论:髂内动脉球囊阻断术应用于凶险性前置胎盘、胎盘植入患者剖宫产中,能够缩短手术时间,减少术中出血量和术后抗生素使用时间,且子宫切除率和术后并发症发生率低,值得进一步应用。
Objective:To analyze the effect of balloon occlusion of internal iliac artery on cesarean section in patients with pernicious placenta previa and placenta accreta.Method:A total of 86 patients with pernicious placenta previa and placenta accreta undergoing cesarean section in our hospital from October 2018 to April 2020 were retrospectively analyzed.They were divided into the control group(n=40,routine cesarean section)and the observation group(n=46,balloon occlusion of the internal iliac artery on the basis of the control group).The operation time,intraoperative blood loss and the use time of antibiotics,the rate of hysterectomy and the incidence of complications in two groups were compared.Result:The operation time and postoperative antibiotic use time of the observation group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group(P<0.05);the hysterectomy rate and the incidence of postoperative complications in the observation group were lower than those in the control group(P<0.05).Conclusion:The application of internal iliac artery balloon occlusion in cesarean section of patients with pernicious placenta previa and placenta accreta can short the operation time and reduce intraoperative blood loss and postoperative antibiotic use time.The hysterectomy rate and the incidence of postoperative complications are low,which is worthy of further application.
