美容小切口手术治疗乳腺良性肿瘤的疗效及随访研究

Curative effect and follow-up study of cosmetic small incision surgery in the treatment of benign breast tumor

目的探讨美容小切口手术治疗乳腺良性肿瘤患者的疗效及近期随访。方法选取2017年1月-2019年1月河南大学第一附属医院收治的224例女性乳腺良性肿瘤患者,依据手术方式不同分为观察组和对照组,观察组采用经乳晕小切口或乳房下皱襞切口手术,对照组患者在乳腺肿瘤表面皮肤行放射状切口手术,对比分析两组手术情况、乳房美观度(乳房外形、皮肤色素、乳晕区感觉、瘢痕)、并发症及术后乳房外形主观满意度。结果观察组手术时间(Operation Time)、伤口愈合时间(Wound Healing Time)、术中出血量(Intraoperative Blood Loss)与对照组相比,差异无统计学意义(P > 0.05),观察组在乳房局部皮肤色素沉着、瘢痕及术后乳房外形的评分均高于对照组(P<0.05),观察组患者并发症发生率6.25%(7/112)低于对照组11.61%(13/112),乳房外形满意度98.21%(110/112)高于对照组90.18%(101/112),差异有统计学意义(P<0.05)。结论乳腺良性肿瘤采用美容小切口手术治疗效果显著,并发症发生率低,乳房外形美观。

Objective To investigate the curative effect of areola incision in the treatment of breast fibroadenoma and shortterm follow-up results.Methods 224 breast fibroadenoma were selected according to different surgical methods from Jan 2017 to Jan 2019 in our hospital,112 cases in each group.The study group was treated with areola incision surgery,the control group was treated with traditional radial incision surgery.After 1 years of follow-up,we compared two groups of statistical operation,the scar and breast surface appearance(degree of pigmentation,scar,breast shape,nipple and areola),incidence of complications,satisfaction of breast appearance.Results Operation:the blood loss and wound healing time in the study group were more than those in the control group,the difference was statistically significant(P>0.05);Breast surface appearance score:the study group of pigmentation,scar,breast shapewere higher than those in the control group,the difference was statistically significant(P<0.05);Complication rate:the complication rate of the study group 6.25%(7/112)was lower than that of the control group 11.61%(13/112),the difference was statistically significant;Satisfaction of breast aesthetic effect:the satisfaction of breast aesthetic effect of study group 98.21%(110/112)was higher than that of control group 90.18%(101/112),the difference was statistically significant(P<0.05).Conclusion The effect of the ring areola incision in the treatment of breast fibroadenoma significantly,can reduce the operation time and blood loss,less postoperative scar,can effectively improve breast appearance,and a lower incidence of complications,worthy of promotion.
