CGF联合Bio-oss骨代材料在下颌阻生智齿致第二磨牙远中骨缺损的应用研究 被引量:5

摘要 目的探讨浓缩生长因子(Concentrate Growth Factors,CGF)联合Bio-oss骨代材料在下颌阻生智齿致第二磨牙远中骨缺损中的应用效果。方法选取2018年1月~2019年10月于本院就诊的200例下颌阻生智齿致第二磨牙远中骨缺损的患者为研究对象。根据随机数表法分成观察组(100例)与对照组(100例)。观察组给予CGF联合Bio-oss骨代材料治疗,对照组采用Bio-oss骨代材料治疗。比较两组患者治疗后牙周袋深度PD值、第二磨牙牙槽嵴至釉牙本质处距离BC值、新生骨密度、术后症状严重度量表及邻牙松动、敏感发生率。结果两组患者治疗后3个月、6个月的PD值明显低于治疗前(P<0.05);观察组患者治疗后3个月的PD值明显低于同期对照组(P<0.05),但观察组治疗后6个月的PD值与同期对照组无显著性差异(P>0.05)。两组患者治疗后3个月、6个月的颊轴角、舌轴角、牙槽嵴顶BC值明显低于治疗前(P<0.05);观察组患者治疗后3个月、6个月的颊轴角、舌轴角、牙槽嵴顶BC值明显低于同期对照组(P<0.05)。观察组治疗后1个月、3个月、6个月的新生骨骨密度明显高于对照组(P<0.05)。观察组治疗后PoSSe饮食、感觉、言语、疼痛、外观、恶心度、日常生活及总分明显低于对照组(P<0.05)。两组患者邻牙松动、敏感总发生率明显高于对照组(P<0.05)。结论 CGF联合Bio-oss骨代材料治疗下颌阻生智齿致第二磨牙远中骨缺损疗效确切,能有效促进第二磨牙远中骨恢复,减少邻牙松动、敏感,具有较高的临床应用价值。 Objective To investigate the application effect of Concentrate Growth Factors(CGF)combined with Bio-oss bone materials in the distal molar defect of the second molar caused by mandibular impacted wisdom teeth,and provide reference for clinical treatment.Methods 120 patients with distal mandibular defects caused by mandibular impacted wisdom teeth from January 20 to December 2018 were enrolled in the study.According to the method of random number table,the patients were divided into observation group(60 cases)and control group(60 cases).Both groups underwent mandibular impacted wisdom tooth extraction.The observation group was treated with CGF combined with Bio-oss bone substitute material,and the control group was only treated with Bio-oss bone substitute material.The depth PD of the periodontal pocket after treatment,the BC value of the second molar alveolar ridge to the enamel dentin,the new bone mineral density,the postoperative symptom severity scale,and the incidence of adjacent tooth loosening and sensitivity were compared.Results The PD values of the two groups at 3 months and 6 months after treatment were significantly lower than those before treatment(P<0.05).The PD value of the observation group was significantly lower than that of the control group at 3 months after treatment(P<0.05).However,there was no significant difference between the observation group and the control group at 6 months after treatment(P>0.05).The BC values of the cheek axis,tongue angle and alveolar crest were significantly lower in the two groups at 3 months and 6 months after treatment(P<0.05).The observation group was 3 months and 6 months after treatment.The BC values of the cheek axis,tongue angle and alveolar crest were significantly lower than the control group(P<0.05).The bone mineral density of the new bone at 1 month,3 months,and 6 months after treatment was significantly higher in the observation group than in the control group(P<0.05).PoSSe diet,sensation,speech,pain,appearance,nausea,daily life and total score were significantly lower in the observation group than in the control group(P<0.05).The incidence of adjacent teeth loosening and sensitivity in the two groups was significantly higher than that in the control group(P<0.05).Conclusion CGF combined with bio OSS bone substitute material is effective in the treatment of distal and middle bone defects of the second molar caused by impacted mandibular wisdom teeth.It can promote the recovery of distal and middle bone of the second molar,reduce the incidence of adjacent tooth loosening and sensitivity,and has high clinical application value.
