摘要 世界第一例植入式心脏起搏器于1958年在瑞典的斯德哥尔摩Karolinska医院植入,从此开创了心脏病领域起搏器治疗的新纪元。其后经过60多年的努力,心脏起搏器领域今非昔比,战果辉煌。从单腔起搏器到双腔起搏器至三腔起搏器,从非生理性到半生理性至生理性起搏,从治疗心动过缓到心动过速至心力衰竭的同步化治疗,从远离核磁到磁共振兼容,从单位点起搏到多位点起搏。起搏器体积越来越小,外形越来越圆滑,电池寿命越来越长,功能越来越智能化。起搏电极导线也有明显进步,导线越来越细,抗压性及抗磨损性越来越强,起搏极性越来越多(从单级到双极至四级),起搏向量组合越来越广(数十种向量组合)。虽然如此,电极导线还是有一些无法避免的并发症。如:导线磨损及断裂,起搏器囊袋破溃和感染,穿刺血管的血栓及堵塞及疤痕局部疼痛等。更有甚者是电极导线所致的大量三尖瓣反流,致使患者右心功能急剧恶化,乃至死亡。
