颈内动脉血泡样动脉瘤的诊治1例报告并文献复习

Diagnosis and treatment of internal carotid artery blister-like aneurysm(report of one case and review literature)

目的研究颈内动脉血泡样动脉瘤的诊断与治疗。方法回顾性分析1例颈内动脉床突上段血泡样动脉瘤患者的临床资料;并进行相关文献复习。患者入院后DSA示右侧颈内动脉动脉瘤,行单纯弹簧圈栓塞治疗;术后1个月复查DSA符合血泡样动脉瘤的诊断,再行开颅动脉瘤包裹夹闭术。结果患者血泡样动脉瘤包裹夹闭术后无神经功能障碍;8个月后的随访复查DSA示脉瘤完全夹闭,载瘤动脉通畅。结论血泡样动脉瘤的诊断标准应适度放宽,无论采用何种血管内治疗方法,术后均要尽早复查。开颅动脉瘤包裹夹闭术的操作相对简单、快捷、经济,是治疗血泡样动脉瘤的有效方法。

Objective To analyze the diagnosis and treatment for the blister-like aneurysm of internal carotid artery.Methods The clinical data of one patient with blister-like aneurysm on the superior clinoid segment of the internal carotid artery were analyzed retrospectively and the relevant literature were reviewed.After admission,DSA showed the right internal carotid artery aneurysm,which was treated with simple coil embolization.One month after operation,DSA was reexamined to meet the diagnosis of bubble like aneurysm,and then the intracranial aneurysm was wrapped and clipped.Results There was no neurological defect after clip-reinforced wrapping of the aneurysms.The results of follow-up angiography after 8 months were satisfactory with the patency of the arteries.Conclusions The diagnostic criteria of blister-like aneurysm should be moderately expanded.No matter what kind of endovascular therapy is used,early follow-up should be done after the procedure.Wrap-clipping craniotomy for blood blister-like aneurysm is relatively simple,quick,economical and effective,and the flow diverters may has a promising application prospect.
