西格列汀联合甘精胰岛素对2型糖尿病患者血糖、血清瘦素及胰岛素抵抗的影响

Effect of Sitagliptin combined with Insulin Glargine on blood glucose,serum leptin and insulin resistance in patients with type 2 diabetes mellitus

目的探讨西格列汀联合甘精胰岛素对2型糖尿病患者血糖、血清瘦素及胰岛素抵抗的影响。方法选取2018年3月~2019年12月南昌大学第三附属医院收治的72例2型糖尿病患者作为研究对象,采用随机数字表法将其分为对照组与观察组,每组各36例。对照组患者皮下注射甘精胰岛素,观察组患者在此基础上加用西格列汀,两组均治疗2个月。观察两组患者治疗前后的血糖、血清瘦素水平、胰岛功能及不良反应发生情况。结果治疗2个月后,两组患者糖代谢指标、血清瘦素及空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组患者的不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论西格列汀联合甘精胰岛素对2型糖尿病患者进行治疗,可有效降低患者血糖、血清瘦素水平,改善患者胰岛素抵抗,减少不良反应的发生,具有较好的治疗效果。

Objective To investigate the effect of Sitagliptin combined with Insulin Glargine on blood glucose,serum leptin and insulin resistance in patients with type 2 diabetes mellitus.Methods A total of 72 patients with type 2 diabetes mellitus who were admitted to the Third Affiliated Hospital of Nanchang University from March 2018 to December 2019 were selected as the research objects and divided into the control group and the observation group according to the random number table method,with 36 cases in each group.The control group was given subcutaneous injection of Insulin Glargine,and the observation group was treated with Sitagliptin based on the control group,with 2 months of treatment in two groups.The blood glucose,serum leptin levels,pancreatic function before and after treatment and incidence of adverse reactions in the two groups were observed.Results After treatment for 2 months,the levels of glucose metabolism indexes,serum leptin,fasting insulin(FINS)and homeostasis model assessment of insulin resistance(HOMA-IR)in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence rate of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Sitagliptin combined with Insulin Glargine in the treatment of patients with type 2 diabetes mellitus can decrease the levels of blood glucose and serum leptin,improve insulin resistance,and reduce the incidence of adverse reactions,which has better curative effect.
