解热止咳散联合阿奇霉素治疗小儿支原体肺炎痰热闭肺证51例临床观察

Clinical observation on 51 cases of mycoplasma pneumonia in children with phlegm heat obstructing lung syndrome treated by Jiere Zhike San(解热止咳散)combined with azithromycin

目的观察解热止咳散联合阿奇霉素治疗小儿支原体肺炎痰热闭肺证的临床疗效。方法选取2018年7月至2019年7月绍兴市中心医院儿科收治的支原体肺炎痰热闭肺证患儿102例,按随机数字表法分为治疗组和对照组,各51例。对照组采用静脉滴注阿奇霉素治疗,治疗组在对照组治疗方法的基础上联合解热止咳散口服,2组均连续用药1周后,比较2组的临床疗效和治疗前后临床症状(发热、咳嗽、咳痰、肺部湿啰音)评分以及不良反应发生情况。结果治疗组总有效率为90.20%(46/51),显著高于对照组的78.43%(40/51),2组比较,差异有统计学意义(P<0.05)。治疗前,2组临床症状积分比较,差异均无统计学意义(P>0.05),具有可比性;治疗后,2组临床症状积分均较同组治疗前显著下降(P<0.05),且治疗组下降更显著,差异有统计学意义(P<0.05)。2组均各有2例患儿出现胃肠道反应,停药后症状消失。结论解热止咳散联合阿奇霉素治疗小儿支原体肺炎痰热闭肺证临床疗效满意,可显著改善患儿的临床症状,值得临床推广应用。

Objective To observe the clinical efficacy of Jiere Zhike San(解热止咳散)combined with azithromycin in the treatment of mycoplasmal pneumonia in children with phlegm heat obstructing lung syndrome.Methods From July 2018 to July 2019,102 cases of mycoplasmal pneumonia in children with phlegm heat obstructing lung syndrome admitted to pediatrics department of Shaoxing Central Hospital were divided into treatment group and control group according to random number table method,with 51 cases in each.The control group was treated with azithromycin intravenously,on this basis,the treatment group was combined with oral administration of Jiere Zhike San.After continuous treatment for 1 week,the clinical efficacy,clinical symptom scores of fever,cough,expectoration,lung wet rale and adverse reactions of the 2 groups before and after treatment were compared.Results The total effective rate of the treatment group was 90.20%(46/51),which was significantly higher than 78.43%(40/51)of the control group,and the difference was statistically significant between the 2 groups(P<0.05).Before treatment,the difference was not statistically significant in clinical symptom scores of fever,cough,expectoration and lung wet rale between the 2 groups(P>0.05),and they were comparable.After treatment,the above-mentioned scores of the 2 groups all significantly decreased compared with the same group before treatment(P<0.05),moreover,the treatment group decreased more significantly,and the difference was statistically significant(P<0.05).There were 2 cases of gastrointestinal reactions in both groups,and the symptoms disappeared after discontinuation.Conclusion It has satisfactory clinical efficacy and can markedly improve the clinical symptoms of children patients to use Jiere Zhike San combined with azithromycin in the treatment of mycoplasmal pneumonia in children with phlegm heat obstructing lung syndrome,which deserves clinical promotion and application.
