摘要
目的:调查新生儿重症监护室(NICU)医护人员对家庭参与式照护(FICare)态度的现况,以期为FICare的临床开展提供参考依据。方法:2019年3—4月使用分层抽样法,选取国内31所三甲儿童专科医院NICU医护人员,按照医生、护士2∶8比例,通过在线平台发放自制的FICare态度问卷,分析不同特征NICU医护人员对FICare态度得分的差异和影响因素。本研究共发放问卷310份,回收有效问卷298份,有效回收率为96.13%。结果:298名NICU医护人员FICare态度问卷重要性维度得分为(6.25±0.87)分,可行性维度为(2.76±0.53)分。单因素分析结果显示,已婚人员、长白班医护人员FICare态度问卷重要性维度、可行性维度得分高于未婚人员和白夜倒班医护人员,差异均有统计学意义(P<0.05)。多重线性回归结果显示,职称是NICU医护人员对FICare态度可行性维度得分的影响因素(β'=0.217,P<0.01)。结论:NICU医护人员认为FICare开展有利于患儿健康恢复及家庭支持,NICU管理层对其推行持肯定态度。建议结合医护人员FICare态度相关影响因素选拔人员、组建团队、开展针对性培训,制订切实可行的FICare实施流程和方案,促进该模式在临床开展。
Objective To investigate the current status of beliefs and attitudes in family integrated care(FICare)of medical staff in Neonatal Intensive Care Unit(NICU)in order to provide a reference for the clinical development of FICare.Methods From March to April 2019,stratified sampling method was used to select the medical staff of NICU in 31 ClassⅢGrade A children's specialized hospitals in China.According to the ratio of 2 to 8 for doctors and nurses,the self-designed FICare Attitude Questionnaire was distributed through the online platform to analyze the differences and influencing factors of FICare beliefs and attitudes scores of NICU medical staff with different characteristics.In this study,a total of 310 questionnaires were issued and 298 valid questionnaires were recovered,with an effective recovery rate of 96.13%.Results The score of importance dimension of FICare Attitude Questionnaire of 298 NICU medical staff was(6.25±0.87)and the score of feasibility dimension of FICare Attitude Questionnaire was(2.76±0.53).Univariate analysis results showed that the scores of importance dimension and feasibility dimension of FICare Attitude Questionnaire were higher for the married and the day-time workers than for the unmarried and the night-day shift staff,and the differences were statistically significant(P<0.05).Multiple linear regressions showed that professional title was a significant factor in the score of feasibility dimension of FICare Attitude Questionnaire of NICU medical staff(β'=0.217,P<0.01).Conclusions NICU medical staff believe that FICare is conducive to the health recovery and family support of children,and the NICU managers hold a positive attitude towards its implementation.It is suggested that a feasible FICare implementation process and program should be developed in combination with the influencing factors related to FICare beliefs and attitudes of medical staff,such as personnel selection,team building and targeted training,so as to promote the development of this mode in clinical practice.
