摘要
目的:探讨分析大剂量重组人生长激素(r-hGH)治疗特发性矮小儿童的临床效果。方法:选取2015年6月至2019年7月收治的80例特发性矮小儿童,分为A、B组,均给予重组人生长激素治疗,A组大剂量[0.2 IU/(kg·d)],B组小剂量[0.1 IU/(kg·d)],比较两组治疗前后发育相关指标、血清胰岛素样生长因子1(IGF-1)、胰岛素样生长因子结合蛋白-3(IGFBP-3)、骨代谢指标水平,并分析治疗过程中的不良反应发生情况。结果:治疗前两组发育相关指标、血清IGF-1、IGFBP-3、骨代谢指标差异无统计学意义(P>0.05),治疗后A组身高、体重、生长速度(GV)、身高标准差积分(Ht SDS)、血清IGF-1、IGFBP-3、骨碱性磷酸酶(BAP)指标改善更显著(P<0.05),两组不良反应发生率差异无统计学意义(P>0.05)。结论:大剂量[0.2 IU/(kg·d)]r-hGH治疗特发性矮小症患儿促进患儿生长效果显著,刺激骨细胞分裂增殖,有助于骨骼的形成,安全、有效。
Objective:To explore and analyze the clinical effects of high-dose recombinant human growth hormone in the treatment of idiopathic short stature children.Methods:80 cases of idiopathic short stature children admitted from June 2015 to July 2019 were selected and divided into group A and B,both of which were given recombinant human growth hormone therapy.The group A was given high dose 0.2 U/(kg·d),and the group B low dose 0.1 U/(kg·d).The related index,serum IGF-1,IGFBP-3,and bone metabolism index levels were compared in the two groups before and after treatment,and the occurrence of adverse reactions during treatment were analyzed.Results:There was no significant difference in development-related indexes,serum IGF-1,IGFBP-3,and bone metabolism indexes between the two groups before treatment(P>0.05);after treatment the height,weight,GV,Ht SDS,serum IGF-1,IGFBP-3,and BAP index in group A improved more significantly(P<0.05);there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:High dose(0.2 U/kg/d)r-hGH in the treatment of children with idiopathic short stature has a significant effect on promoting the growth of children,stimulating the division and proliferation of bone cells,and contributing to bone formation,which is safe and effective.
