急性白血病化疗患者肛周感染发生状况及其影响因素

Perianal infection and its influencing factors in patients with acute leukemia undergoing chemotherapy

目的研究急性白血病化疗患者肛周感染发生状况及其影响因素。方法选取2017年2月至2019年1月深圳市第二人民医院急性白血病化疗患者498例,采用回顾性分析方法观察患者相关资料、对急性白血病化疗肛周感染患者加强监测,统计肛周感染率、肛周感染类型,采用Logistic分析急性白血病化疗患者肛周感染相关性影响因素。结果急性白血病化疗患者肛周感染37例、感染率7.43%,年龄、糖尿病、肛周基础疾病(痔、肛裂史)、便秘或腹泻、白蛋白、血小板计数恢复时间、中性粒细胞恢复时间、住院时间分别与同组类型比较,差异有统计学意义(P<0.05)。Logistic回归分析证实,肛周基础疾病(痔、肛裂史)、便秘或腹泻、血小板计数恢复时间、中性粒细胞恢复时间和住院时间是导致急性白血病化疗患者肛周感染的高危风险因素(P<0.05)。结论急性白血病化疗患者肛周感染率仍处于高位状态,值得引起临床认识,加强感染风险因素控制,利于降低急性白血病化疗患者肛周感染率,提高化疗质量。

Objective To study the incidence of perianal infection in patients with acute leukemia undergoing chemotherapy and its influencing factors.Methods A total of 498 patients with acute leukemia undergoing chemotherapy in this hospital from February 2017 to January 2019 were selected.Retrospective analysis was used to observe the relevant data of the patients.Patients with perianal infection after chemotherapy for acute leukemia were monitored intensively.The perianal infection rate and the type of perianal infection were counted.Logistic analysis was used to analyze the influencing factors of perianal infection in patients with acute leukemia receiving chemotherapy.Results A total of 37 patients with acute leukemia received chemotherapy and the infection rate was 7.43%.Age,diabetes,perianal basic diseases(haemorrhoids and history of anal fissure),constipation or diarrhea,albumin,platelet count recovery time,neutrophil recovery time,and length of hospital stay were compared with the same group,and the differences were statistically significant(P<0.05).Logistic regression analysis confirmed that perianal basic diseases(haemorrhoids and history of anal fissure),constipation or diarrhea,platelet count recovery time,neutrophil recovery time,and length of hospital stay were high risk factors for perianal infection in patients receiving chemotherapy for acute leukemia(P<0.05).Conclusions The perianal infection rate in patients with acute leukemia chemotherapy is still at a high level,which is worthy of clinical recognition.Strengthening the control of infection risk factors is conducive to reducing perianal infection rate and improving the quality of chemotherapy in patients with acute leukemia chemotherapy.
