口腔黏膜下纤维性变及相关口腔癌患者血清SCC-Ag、Cyfra21-1、NSE及CEA的水平变化和意义 被引量:9

Significance of serum SCC-Ag,Cyfra21-1,NSE and CEA levels in patients with oral submucosal fibrosis and OSF-associated oral cancer

摘要 目的:探讨血清鳞状细胞癌抗原(SCC-Ag)、细胞角蛋白19片段(Cyfra21-1)、神经元特异性烯醇化酶(NSE)及癌胚抗原(CEA)在口腔黏膜下纤维性变(OSF)及OSF相关口腔癌(OSF-OSCC)中的表达和意义。方法:纳入2018年8月~2019年12月在华中科技大学同济医学院附属协和医院接受诊治疗的72例OSF患者以及50例健康体检者,并根据OSF是否伴有口腔黏膜癌变,分为OSF组50例和OSF-OSCC组22例。检测不同组别的SCC-Ag、Cyfra21-1、NSE及CEA水平,分析各肿瘤标志物的血清表达水平与疾病临床病理的相关性,探讨血清SCC-Ag、Cyfra21-1、NSE及CEA水平对口腔黏膜下纤维性变病情变化中的意义。结果:3组血清SCC-Ag、Cyfra21-1水平含量差异具有统计学意义(P<0.05),OSF组与健康对照组对比,SCC-Ag水平含量差异具有统计学意义;与OSF组对比,OSF-OSCC组患者血清SCC-Ag、Cyfra21-1水平明显升高(P<0.05);3组患者NSE及CEA水平无显著差异(P>0.05)。结论:在OSF及其相关口腔癌的患者中,体内血清SCC-Ag、Cyfra21-1含量差异较为明显,其检测有利于OSF及其相关口腔癌的早期诊断,提示SCC-Ag、Cyfra21-1与OSF及OSF相关口腔癌的发生发展有一定关联。 Objective:To investigate the significance of serum squamous cell carcinoma antigen(SCC-AG),cytokeratin 19 fragment(Cyfra21-1),neuron-specific enolase(NSE)and carcinoembryonic antigen(CEA)in the changes of oral submucosal fibrosis(OSF)and OSF-associated oral cancer.Methods:Seventy-two patients diagnosed and treated in Union Hospital from August 2018 to December 2019,also 50 healthy physical examination subjects were included.According to the changes of OSF,patients with OSF were divided into OSF lesion group(50 patients)and OSF-related cancer(OSF-OSCC)group(22 patients).The levels of SCC-AG,Cyfra21-1,NSE and CEA in different groups of people were detected to analyze the correlation between serum expression levels of each marker and the clinicopathology of the disease,and to explore the significance of serum SCC-AG,Cyfra21-1,NSE and CEA levels in the changes of oral submucosal fibrosis.Results:There were statistically significant differences in serum SCC-AG and Cyfra21-1 levels between the three groups(P<0.05),and there were statistically significant differences in serum SCC-AG levels between the OSF group and the healthy control group.Compared with the OSF group,the serum levels of SCC-AG and Cyfra21-1 in the OSF-OSCC group were significantly increased(P<0.05).There was no significant difference in NSE and CEA levels among the three groups(P>0.05).Conclusion:Serum SCC-AG and Cyfra21-1 levels were significantly different in patients with OSF and related oral cancer,and their detection was beneficial to the early diagnosis of OSF and related oral cancer,suggesting that SCC-AG and Cyfra21-1 were correlated with the changes of OSF.
