腺苷蛋氨酸联合复方赶黄草方治疗胆道梗阻所致胆汁淤积性肝损伤的疗效分析

Efficacy Analysis of Adenosylmethionine Combined with Compound Ganhuangcao in Treating Cholestatic Liver Injury Caused by Biliary Obstruction

目的:探讨腺苷蛋氨酸联合复方赶黄草方治疗胆道梗阻所致胆汁淤积性肝损伤的临床疗效。方法:选取2018年10月—2019年4月在我院收治的胆道梗阻所致胆汁淤积性肝损伤的患者70例,根据随机数字表法将患者分为对照组和观察组,每组患者35例。其中对照组患者使用丁二磺酸腺苷蛋氨酸肠溶片,观察组患者额外联合复方赶黄草方汤剂,比较两组的疗效、肝功能以及炎性因子的水平。结果:治疗后,两组患者血清中的直接胆红素(DBIL)、总胆固醇(CHOL)、丙氨酸氨基转氨酶(ALT)、天冬氨酸氨基转氨酶(AST)、碱性磷酸酶(ALP)、总胆红素(TBIL)、间接胆红素(IBIL)水平均显著降低(P<0.05),且观察组优于对照组;两组患者白细胞计数(WBC)、C反应蛋白(CRP)表达水平均显著降低(P<0.01),同时观察组下降水平较对照组更加明显(P<0.05),而两组患者血清IgA、IgE、Ca2+水平与治疗前比差异无统计学意义(P>0.05)。结论:腺苷蛋氨酸联合复方赶黄草方可以有效缓解胆道梗阻所致胆汁淤积性肝损伤临床症状,能够改善患者肝功能并降低炎症反应水平。

Objective To investigate the clinical effect of adenosylmethionine combined with compound herbal medicine on cholestatic liver injury caused by biliary obstruction.Methods A total of 70 patients with cholestatic liver injury caused by biliary obstruction treated in our hospital from October 2018 to April 2019 were selected,and the patients were divided into the control group and the observation group according to the random digital table method.Control group was treated with adenosylmethionine,and the observation group was administrated with compound herbal medicine on the basis of that above.The clinical efficacy,liver function and inflammatory factors of the two groups were compared.Results After treatment,serum bilirubin(DBIL),total cholesterol(CHOL),alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),total bile-The levels of erythroglobin(TBIL)and indirect bilirubin(IBIL)were significantly improved(P<0.05),and the observation group was better than those of the control group.The levels of white blood cell count(WBC)and C-reactive protein(CRP)in the two groups of patients were significantly reduced(P<0.01),while the levels of decline in the observation group were more significant than those of the control group(P<0.05).There were no significant difference in the levels of IgA,IgE and Ca2+in the serum of patients in the control group and the observation group compared with those before treatment(P>0.05).Conclusion Adenosylmethionine combined with compound herbal medicine can effectively alleviate the clinical symptoms of cholestatic liver injury caused by biliary obstruction and it can improve liver function and reduce inflammation level in patients.
