电针联合隔药饼灸治疗脊髓损伤后神经源性膀胱的疗效观察

The Curative Effect Observation of Electroacupuncture Combined with Cake-separated Moxibustion on Neurogenic Bladder after Spinal Cord Injury

目的:观察电针联合隔药饼灸治疗脊髓损伤后神经源性膀胱的临床疗效。方法:选取2016年8月—2019年5月我院收治的72例脊髓损伤后神经源性膀胱患者为研究对象。按随机数字表法分为观察组和对照组,每组36例。对照组在采用膀胱功能训练等常规治疗基础上给予隔药饼灸治疗,观察组在对照组基础上加用电针治疗。治疗4周后,比较两组患者膀胱最大容量、膀胱残余尿量、尿常规阳性率和总有效率。结果:两组患者膀胱最大容量均高于治疗前,且治疗前后差值观察组明显大于治疗组,差异有统计学意义(P<0.05);两组患者膀胱残余尿量均低于治疗前,且治疗前后差值观察组明显大于对照组,差异有统计学意义(P<0.05)。在治疗第3、4周后,观察组患者的尿常规阳性率明显低于对照组,差异有统计学意义(P<0.05)。观察组总有效率为83.33%,明显优于对照组63.89%,差异有统计学意义(P<0.05)。所有患者治疗过程中无不良反应发生。结论:电针联合隔药饼灸治疗脊髓损伤后神经源性膀胱疗效满意,并具有安全、方法简易、易于实施的优点,是一种值得推荐的中医外治法,适合在临床上推广和应用。

Objective To observe the clinical efficacy of electroacupuncture combined with cake-separated moxibustion in the treatment of neurogenic bladder after spinal cord injury.Methods 72 patients with urinary retention after spinal cord injury from August 2016 to May 2019 were selected for the study.All patients were divided into two groups.36 patients in the observation group and 36 patients in the control group according to the random number table method.The control group was treated with cake-separated moxibustion on the basis of routine treatment such as bladder function training.The observation group was treated with electroacupuncture on the basis of control group.After 4 weeks of treatment,the maximum bladder capacity,residual urinary volume,positive rate of urinary routine and total efficiency were compared between the two groups.Results The maximum volume of bladder in both groups was higher than that before treatment(P<0.05),and the difference before and after treatment of observation group was significantly greater than that in the control group(P<0.05).The residual urine volume of bladder in both groups was lower than that before treatment,and the difference before and after treatment of observation group was significantly greater than that in the control group(P<0.05).After 3 and 4 weeks of treatment,the positive rate of urinary routine in the observation group was significantly lower than that in the control group(P<0.05).The total effective rate of the observation group was 83.33%,which was significantly better than 63.89%of the control group(P<0.05).No adverse reactions occurred during the treatment of all patients.Conclusion Electroacupuncture combined with cake-separated moxibustion in the treatment of neurogenic bladder after spinal cord injury is satisfactory.It has the advantages of safety,simple method,which is easy to implement.It is a recommended method of external treatment of traditional Chinese medicine and is suitable for clinical promotion and application.
