银杏叶提取物联合依达拉奉治疗急性脑出血患者疗效观察及对炎症因子的影响

Effect of ginkgo biloba extract combined with edaravone on acute intracerebral hemorrhage and its effect on serum inflammatory factors

目的观察银杏叶提取物联合依达拉奉治疗急性脑出血(AICH)患者临床疗效及对炎症因子的影响。方法选取2018年7月至2019年6月于本院就诊的78例AICH患者,随机分为治疗组和对照组,每组39例,其中对照组予依达拉奉注射液治疗,治疗组予银杏叶提取物注射液联合依达拉奉注射液治疗,两组患者均连续治疗14 d。记录并分析两组患者治疗前后脑血肿与水肿体积、中国脑卒中临床神经功能缺损程度评分量表(CSS)评分、改良Barthel指数(MBI)评分、血清炎症因子[超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]表达水平。结果治疗后,治疗组脑血肿、水肿体积均明显小于对照组(均P<0.05),治疗组CSS评分明显低于对照组(P<0.05)、MBI评分明显高于对照组(P<0.05),治疗组血清炎症因子(hs-CRP、TNF-α、IL-6)表达水平均明显低于对照组(均P<0.05);且治疗组神经功能疗效明显高于对照组(P<0.05)。结论银杏叶提取物联合依达拉奉可有效减少AICH患者脑血肿与水肿体积、改善神经功能、提高日常生活能力,并可抑制血清hs-CRP、TNF-α、IL-6表达水平,疗效和安全性良好,具有临床应用价值。

Objective To investigate the efficacy of ginkgo biloba extract combined with edaravone in the treatment of acute intracerebral hemorrhage(AICH)and its effect on the expression of serum inflammatory factors.Methods 78 patients with AICH in our hospital from July 2018 to June 2019 were randomly divided into the treatment group and the control group,39 cases in each group.The control group was treated with edaravone injection,and the treatment group was treated with ginkgo biloba extract injection combined with edaravone injection,and the two groups were treated for 14 days.Before and after treatment,cerebral hematoma and edema volume,CSS score,modified Barthel index(MBI),expression levels of serum hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)were recorded and analyzed.Results After treatment,cerebral hematoma and edema volume in the treatment group were significantly smaller than those in the control group(both P<0.05),CSS score in the treatment group was significantly lower than that in the control group(P<0.05),MBI in the treatment group was significantly higher than that in the control group(P<0.05),the expression levels of serum inflammatory factors(hs-CRP,TNF-α,IL-6)in the treatment group were significantly lower than those in the control group(all P<0.05),and neurological efficacy in the treatment group was significantly higher than that in the control group(P<0.05).Conclusion Ginkgo biloba extract combined with edaravone can effectively reduce cerebral hematoma and edema volume in patients with AICH,improve the neurological function and the ability of daily life,and inhibit the expression of hs-CRP,TNF-α,IL-6,with good efficacy and safety and clinical application value.
