沙滩椅在肩关节镜术中应用价值研究

Value of beach chair position nursing combined with lateral position traction in patients undergoing shoulder arthroscopy

目的探讨沙滩椅位护理联合侧卧位牵引应用在肩关节镜术中患者的价值。方法选取2016年12月至2020年3月本院行肩关节镜手术患者55例,将其分为对照组(n=23)和试验组(n=32)。对照组实施常规护理联合传统牵引架牵引,试验组实施沙滩椅位护理联合侧卧位牵引。对比两组患者体位安放时间、术中舒适度、手术时间及术中对患者呼吸力学各项指标影响。结果试验组手术时间、术中舒适度及体位摆放时间分别为(103.45±6.38)min、90.63%(29/32)、(14.45±4.32)min,均优于对照组(117.32±8.56)min、69.57%(16/23)、(22.48±5.41)min,差异均有统计学意义(均P<0.05);试验组Peak、CT及Raw指标分别为(16.12±2.41)cmH2O、(61.25±5.03)ml/cmH2O、(14.18±3.34)cmH2O/Ls,均优于对照组(18.34±3.27)cmH2O、(51.23±4.32)ml/cmH2O、(11.24±2.45)cmH2O/Ls,差异均有统计学意义(均P<0.05)。结论将沙滩椅位护理联合侧卧位牵引应用在肩关节镜术缩短患者体位安放时间和手术时间,提高其在术中舒适度,避免术中不良反应的发生,提高手术效率和效果。

Objective To explore the value of beach chair nursing combined with lateral traction in patients undergoing shoulder arthroscopy.Methods Forty-five patients who underwent shoulder arthroscopic surgery in our hospital from December,2016 to March,2020 were divided into a control group(n=23)and an experimental group(n=32).The control group were routinely cared and took traditional traction.The experimental group took beach chair position nursing and lateral recumbent traction.The posture repose time,intraoperative comfort,operation time,and indicators of intraoperative aspiration mechanics were compared between the two groups.Results The operation time,intraoperative comfort,and posture repose time were better in the experimental group than in the control group[(103.45±6.38)min vs.(117.32±8.56)min,90.63%(29/32)vs.69.57%(16/23),and(14.45±4.32)min vs.(22.48±5.41)min;all P<0.05].The Peak,CT,and Raw better in the experimental group than in the control group[(16.12±2.41)cmH2O vs.(18.34±3.27)cmH2O,(61.25±5.03)ml/cmH2O vs.(51.23±4.32)ml/cmH2O,and(14.18±3.34)cmH2O/Ls vs.(11.24±2.45)cmH2O/Ls;all P<0.05].Conclusion The application of beach chair nursing combined with lateral recumbent traction in shoulder arthroscopy can reduce complications,shorten the times of positioning and operation,and improve the operation efficiency and effect.
