摘要
目的:研究超长眼轴白内障人群生物测量值分布及相关性。方法:进行横断面研究,收集2017年~2019年在福建省立医院眼科拟行白内障手术的患者117例179只眼,分别依据患者性别(男或女)、年龄(≤65岁或>65岁)及眼轴(≤30 mm或>30 mm)对患者进行分组,测量其眼轴长度、角膜曲率(角膜平坦子午线曲率K1、角膜陡峭子午线曲率K2、角膜平均曲率K)、角膜散光和前房深度等,研究其生物测量值分布及相关性。采用独立样本t检验、卡方检验、Mann-Whitney U检验及Pearson相关分析对数据进行统计。结果:女性角膜曲率(K1、K2、K)均值均大于男性(均P=0.000),男性前房深度均值大于女性(P=0.004)。不同年龄两组间的角膜曲率(K1、K2、K)、角膜散光、前房深度及眼轴长度比较,差异无统计学意义(P>0.05)。不同眼轴长度两组角膜曲率(K1、K2、K)比较,差异有统计学意义(P=0.034、0.001、0.004),角膜散光及前房深度差异无统计学意义(P>0.05)。相关分析显示角膜平坦子午线曲率K1与角膜陡峭子午线曲率K2呈正相关(r=0.870,P=0.000),眼轴长度与角膜平均曲率K呈负相关(r=-0.195,P=0.009)。前房深度和年龄及眼轴长度无显著相关(P>0.05)。结论:超长眼轴人群角膜曲率与前房深度表现出性别差异,而不同年龄患者各项生物学参数无明显差异。角膜曲率随眼轴增长而降低。
Objective To investigate the ocular biometric parameters and their associations in cataract patients with over-long axial length.Method Cross-sectional survey observational was studied.179 eyes of 117 patients from 2017 to 2019 in Fujian Provincial Hospital were recruited for this study.The cases were divided into groups according to the sex(male/female),ages(≤65/>65)and axial length(≤30 mm/>30 mm).Ocular parameters including axial length(AL),anterior chamber depth(ACD),corneal refractive power(flatkeratometry K1,steepkeratometry K2 and meankeratometry K)and corneal astigmatism(CA)of the patients were measured and analyzed.Results The mean value of corneal refractive power(K1、K2、K)in women was higher than that in men(all P=0.000).The ACD in men was deeper than that in women(P=0.004).The corneal refractive power(K1、K2、K),CA,ACD and AL was uncorrelated with ages(P>0.05).The corneal refractive power(K1、K2、K)was different among two groups axial length(P=0.034,P=0.001,P=0.004).The CA and ACD was uncorrelate d with axial length(P>0.05).Correlative analysis showed that there were positiveIy correlated with K1 and K2(r=0.870,P=0.000).There were negative correlated with AL and K(r=-0.195,P=0.009).ACD was uncorrelated with ages and AL(P>0.05).Conclusion The corneal refractive power and anterior chamber depth showed gender difference in the over-long axial length group,but there was no significant difference in the biological parameters of patients of different ages.The corneal refractive power decreased with the axial growth.
