急诊信息系统在边远地区的应用及实践

Application and Practice of the Emergency Information System in Remote Areas

目的:探讨急诊信息系统在边远地区的应用效果及实践意义。方法:在急诊分诊台、收费处、急诊药房、急诊辅助检查室等空间布局一体化的基础上,2016年9月起喀什地区第一人民医院设计并应用包括预检分诊、病史采集、检查结果自动获取、医嘱开具、医嘱校对、药物配方发放、护士执行医嘱和急诊会诊等模块在内的一体化急诊信息管理系统,并对应用急诊信息系统前后的急诊患者接诊量、投诉率、医疗纠纷发生率、患者抢救及时率、急救中心内死亡率等指标进行了对比。结果:应用急诊信息系统后,一方面急诊患者接诊量明显增多,患者抢救及时率明显提高,而投诉率、医疗纠纷发生率、急救中心内死亡率等不良事件发生率明显降低;另一方面,急诊流程简化,医护人员文案负担减小,诊区拥堵现象缓解,提升了质量控制和科室管理的水平。结论:急诊信息系统的上线不仅使急诊业务流程更加规范化,缩减了地区医疗水平与急危重患者标准化诊疗规范的差距,还很大程度上提升了医护人员的工作效率和患者的就诊体验。

Objective:To explore the application effect and practical significance of the emergency information system in remote areas.Methods:Based on the integration of the spatial layout of emergency reception desk,charging office,emergency pharmacy and auxiliary examination room of emergency,since September 2016,the First People’s Hospital of Kashgar has designed and applied the integrated emergency information management system,including pre-examination and reception,medical history collection,automatic acquisition of examination results,issuing of medical orders,proofreading of medical orders,distribution of drug formulation,nurses’execution of medical orders,emergency consultation and other modules;and compared the number of emergency patients,complaint rate,the incidence of medical disputes,timely rescue rate of patients,mortality in the emergency center and other indicators before and after the application of the emergency information system.Results:After the application of the emergency information system,on the one hand,the number of emergency patients increased significantly,the timely rescue rate of patients was significantly improved,and the incidence of adverse events such as complaint,medical dispute and death in the emergency center was significantly reduced.On the other hand,the emergency process was simplified,the medical staffs’writing burden was reduced,the congestion in the diagnosis area was relieved,and the level of quality control and department management was improved.Conclusion:The launch of the emergency system not only made the emergency business process more standardized,reduced the gap between the regional medical level and the standardized diagnosis and treatment standard for critically ill patients,but also greatly improved the work efficiency of medical staffs and the experience of patients.
