基于超声血流向量成像技术的冠心病患者左心室舒张期流场状态的评价 被引量:2

Evaluation of the State of the Left Ventricular Diastolic Flow Field in Patients with Coronary Heart Disease Based on Vector Flow Mapping Technology

摘要 目的:分析超声血流向量成像(Vector Flow Mapping,VFM)技术评价冠心病患者左心室舒张期流场状态的价值。方法:纳入冠心病、健康志愿者各70例,为研究组与对照组。测量其常规超声心动图参数及左心室舒张期涡流相关参数,进行统计学分析。结果:与对照组相比,研究组左心室舒张末期容积(LVEDV)、收缩末期容积(LVESV)明显较高,左室射血分数(LVEF)明显较低,且Tei指数显著较高(P<0.05),且左心室舒张中期涡流横径、纵径明显较大,圈数更多(P<0.05);而无症状性心肌缺血型、心力衰竭/心律失常型与心绞痛型、心肌梗死型上述指标相比有统计学差异(P<0.05)。结论:VFM技术可直观显示不同类型冠心病患者左心室舒张期腔内血液流场状态结构,对初步定量评价左心室腔内血流动力学及左心室功能有重要价值。 Objective:To investigate the application value of ultrasound vector flow mapping(VFM)in the evaluation of left ventricular diastolic flow field in patients with coronary heart disease(CHD).Methods:70 cases of coronary heart disease and 70 healthy volunteers were included in the study group and the control group.The parameters of conventional echocardiography and left ventricular diastolic eddy current were measured and analyzed statistically.Results:Compared with the control group,the left ventricular end-diastolic volume(LVEDV)and end-systolic volume(LVESV)of the study group were significantly higher,the left ventricular ejection fraction(LVEF)was significantly lower,and the Tei index was significantly higher(P<0.05),And the left ventricular mid-diastolic vortex diameter and longitudinal diameter were significantly larger,with more turns(P<0.05);and the above indicators of asymptomatic myocardial ischemia,heart failure/arrhythmia,angina and myocardial infarction.There is a significant difference in comparison(P<0.05).Conclusion:VFM technology can visually display the structure of left ventricular diastolic flow field in patients with different types of coronary heart disease.It has important value for preliminary quantitative evaluation of left ventricular hemodynamics and left ventricular function in patients with coronary heart disease.
