摘要
目的:探究二维超声联合高频彩超鉴别乳腺癌腋窝淋巴结(LN)转移的应用价值。方法:选取2015年3月至2017年2月间来我院就诊的乳腺癌患者共84例,分别采用二维超声和高频彩超观察乳腺肿块、腋窝淋巴结,详细记录腋窝淋巴结的大小、回声及血流情况,和术后病理结果进行对比。结果:本组患者均行术后病理检查,确诊58例腋窝淋巴结转移,未转移26例;经常规二维超声敏感度为72.41%,特异度为92.31%,二维超声联合高频彩超敏感度、特异度分别为96.55%、100%,且后者敏感度显著高于前者,P<0.05。结论:二维超声联合高频彩超鉴别乳腺癌腋窝淋巴结转移具有特征性声像图,能够准确判断淋巴结转移情况。
Objective:To explore the value of two-dimensional ultrasonography combined with high frequency ultrasonography in the differential diagnosis of axillary LN metastasis in breast cancer.Methods:A total of 84 patients with breast cancer who were admitted to our hospital from March 2015 to February 2017 were selected,two-dimensional ultrasonography and high-frequency ultrasonography were performed to observe the breast mass and axillary lymph nodes.The size,echo and blood flow of the axillary lymph nodes were recorded in detail,and the pathological changes were compared.Results:The patients underwent postoperative pathological examination,58 cases of axillary lymph node metastasis,26 cases were not transferred.The sensitivity of conventional two-dimensional ultrasonography was 72.41%,the specificity was 92.31%,the positive predictive value of lymph node metastasis was 95.45%and the negative predictive value was 60.00%,the sensitivity,specificity,positive predictive value and negative predictive value of two-dimensional ultrasonography were 96.55%,100%,100%and 92.86%respectively,and the sensitivity and negative predictive value of the two groups were significantly higher than those of the former,P<0.05.Conclusion:Two-dimensional ultrasonography combined with high-frequency ultrasonography can differentiate axillary lymph node metastasis from breast cancer with characteristic ultrasonography,which can accurately determine lymph node metastasis and high application value.
