血清AMH联合性激素对不孕症患者卵巢储备功能的评估价值探析

Analysis of the evaluation value of serum AMH combined with sex hormone on ovarian reserve function of infertility patients

目的:探讨血清抗缪勒管激素(AMH)联合性激素对不孕症患者卵巢储备功能的评估价值。方法:回顾性收集2018年8月~2019年8月我院收治的80例不孕症患者临床资料,所有患者入院后均采用阴道彩超检测其窦卵泡数目(AFC),根据AFC检查结果,对患者卵巢储备功能进行评估,将AFC数目为5~15个,设为卵巢功能正常反应组(n=39),AFC数目<5个,设为卵巢功能减低组(n=41),比较两组性激素水平,并分析卵巢储备功能与血清AMH联合性激素的关系。结果:卵巢低反应组AMH、LH水平较卵巢正常组低,E2、FSH水平高,差异有统计学意义(P<0.05);绘制ROC曲线结果显示,血清AMH联合性激素用于预测不孕症患者卵巢储备功能的曲线下面积为0.816,具有一定预测价值。结论:血清AMH与性激素水平与不孕症患者卵巢储备功能密切相关,临床可根据性激素水平对患者卵巢储备功能进行评估,为临床诊断、治疗提供参考依据,值得推广应用。

Objective:To evaluate the value of serum anti-Mullerian hormone(AMH)combined with sex hormones in infertile patients with ovarian reserve function.Methods:The clinical data of 80 infertile patients who were received by our hospital from August 2018 to August 2019 were collected retrospectively.After admission,all patients were tested for the number of sinus follicles(AFC)by vaginal color Doppler ultrasound.Based on the results of the AFC test,the patients'ovarian reserve function was evaluated.Set the number of AFC to 5-15 as the normal response group of ovarian function(n=39),the number of AFC<5 as the reduced ovarian function group(n=41),compared the sex hormone levels of the two groups,and analyzed the relationship between the ovarian reserve function and serum AMH combined with sex hormones.Results:The levels of AMH and LH in the low ovarian response group were lower than those in the normal ovarian group,and the levels of E2 and FSH were higher,the difference was statistically significant(P<0.05).The results of ROC curve drawing showed that the area under the curve used by serum AMH combined with sex hormones to predict ovarian reserve function in infertile patients was 0.816,which had certain predictive value.Conclusion:Serum AMH and sex hormone levels are closely related to the ovarian reserve function of infertile patients.The clinical evaluation of the patient's ovarian reserve function according to the level of sex hormones can provide a basis for clinical diagnosis and treatment,and is worthy of popularization and application.
