阿替普酶与尿激酶溶栓治疗急性心肌梗死患者的疗效比较

Effect Comparison of Thrombolysis with Alteplase and Urokinase in the Treatment of Acute Myocardial Infarction

目的比较阿替普酶与尿激酶溶栓治疗急性心肌梗死患者的疗效。方法选取2017年5月至2018年4月沈阳市辽中区近海济康医院收治的80例急性心肌梗死患者,将其随机分为对照组和观察组,每组40例。对照组实施尿激酶溶栓治疗,观察组采取阿替普酶溶栓治疗。比较两组患者的治疗效果、病情改善时间、心功能指标及并发症发生情况。结果观察组左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)指标均高于对照组(均P <0.05)。观察组显效32例,有效8例,总有效率为100.00%;对照组显效16例,有效14例,无效10例,总有效率为75.00%;观察组治疗总有效率高于对照组(P <0.05)。观察组急性心肌梗死病情改善时间短于对照组(P <0.05)。观察组并发症发生率低于对照组(P <0.05)。结论阿替普酶溶栓治疗急性心肌梗死的疗效确切,可改善患者的心功能,减少并发症的发生,有效性和安全性均较高。

Objective To investigate the efficacy of thrombolysis with alteplase and urokinase in the treatment of acute myocardial infarction(AMI).Methods From May 2017 to April 2018,80 patients with acute myocardial infarction in our hospital were selected.The patients were randomly divided into two groups:the control group was treated with urokinase thrombolysis,and the observation group was treated with alteplase thrombolysis.The curative eff ect of acute myocardial infarction,the average time of improvement,the monitoring indexes of LVEDD,LVESD and LVEF before and after treatment,and the incidence of complications were compared between the two groups.Results The left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),and left ventricular ejection fraction(LVEF)indexes of the observation group were higher than those of the control group(all P<0.05).In the observation group,32 cases were markedly eff ective,8 cases were eff ective,and the total eff ective rate was 100.00%;the control group was markedly eff ective in 16 cases,eff ective in 14 cases,and ineff ective in 10 cases,and the total eff ective rate was 75.00%;the observation group had a higher total eff ective rate than the control group(P<0.05).The improvement time of acute myocardial infarction in the observation group was shorter than that in the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion Alteplase thrombolysis is eff ective in the treatment of acute myocardial infarction,which can improve the cardiac function of patients and reduce the incidence of complications.It is eff ective and safe.
