摘要
目的探讨紫癜型玫瑰糠疹的临床特点,以提高对该病的认识。方法回顾性分析北京博爱医院皮肤科2019年7月29日收治的1例紫癜型玫瑰糠疹患者的临床资料和检查结果等,并复习相关文献进行分析总结。结果患者先于左大腿伸侧起一前驱斑,后多发紫癜性瘀斑沿皮纹分布,皮疹形态与典型玫瑰糠疹不同,呈出血性瘀斑。组织病理表现为非特异性皮肤炎症伴血管及毛囊周围较多红细胞外溢,不伴有血管炎。治疗一个月,患者无新发皮疹,少许皮疹色泽变暗。两个月后,患者皮疹无增多,部分皮疹色泽变暗,未完全消退。半年后,皮疹已消退。结论紫殿型玫瑰糠疹皮疹表现多样。病程多在3~6个月,口服抗组胺药、外用激素药膏有效,预后较好。
Objective To study the clinical characteristics of Purpuric Pityriasis Rosea and improve the recognition of the disease.Methods To retrospectively analyze the clinical data and reviewing results of 1 patient with pityriasis rosea purpura admitted to the Department of Dermatology of Beijing Boai Hospital on July 29,2019.Related literature was analyzed and summarized.Results A purpuric macule was firstly presented on the front side of the left thigh,followed by multiple purpuric macules along the dermatoglyph,and the presentation of the lesions were different from the typical Pityriasis rosea.The histopathological finding was non-specific skin inflammation accompanied by extravasation of erythrocytes without any evidence of capillaritis or vasculitis.After one month of treatment,the patient had no new rashes and a few rashes turned dark.Two months later,the patient's rash did not increase,and part of the rash became darker and did not completely resolve.Six months later,the rash has subsided.Conclusion Purpuric Pityriasis Rosea are diverse in the distribution or morphology.The course of the disease is mostly between 3 to 6 months.Oral antihistamine and topical hormone ointment are effective,and the disease has a good prognosis.
