复发性流产患者APLA检测及临床应用价值

Detection and clinical application value of antipatemal lymphocyte antibody in patients with recurrent spontaneous abortion

目的探讨抗丈夫淋巴细胞抗体(APLA)检测及临床应用价值。方法回顾性分析320例复发性自然流产患者APLA检测方法,并对淋巴细胞免疫治疗后患者妊娠结局进行比较。结果APLA的检测方法多样,目前最常采用的是酶联免疫吸附法,该方法患者未经干预时阳性检测率为3.98%,对照组为2.89%,两组比较差异无统计学意义(P>0.05);淋巴细胞免疫治疗后APLA阳性率增加为96.4%,其中186例患者妊娠后,18例再次发生流产,流产率为9.70%。结论APLA检测作为判断流产原因的生物学标志物证据不足,其临床应用价值值得进一步研究。

Objective To investigate the clinical application value of antipatemal lymphocyte antibody(APLA).Methods Retrospective analysis on APLA in 320 patients with recurrent spontaneous abortion were performed.And the lymphocyte immunotherapy was also analyzed.Results There were various methods for the detection of APLA.There was no uniform standard in each hospital.The enzyme linked immunosorbent assay(ELISA)was the most common used method.The positive detection rate of the APLA was 3.98%in the patients without intervention,and 4.35%in the control group,while there was no significant difference between the two groups(P>0.05).The rate rose to 96.4%after lymphocyte immunotherapy.Eighteen cases miscarried again in 186 of pregnancy,with the abortion rate of 9.70%.Conclusion It is insufficient to diagnose the cause of abortion by APLA detection.The value of clinical application needs further study.
