糖尿病足患者血清淀粉样蛋白A和脂蛋白相关磷脂酶A2的表达及意义

Expression and significance of serum SAA and lipoprotein-associated phospholipase A2 in diabetic foot patients

目的:分析糖尿病足(DF)患者血清淀粉样蛋白A(SAA)和脂蛋白相关磷脂酶A2(Lp-PLA2)的变化及意义。方法:选取2型糖尿病(DM)者524例,按DF病变是否发生分为DF组(n=256)和DM组(n=268),另择健康者作为对照组(n=270)。比较三组受试者血清SAA和Lp-PLA2水平变化,DF患者按下肢动脉斑块、狭窄及闭塞情况分组分析血清SAA和Lp-PLA2水平,探讨SAA和Lp-PLA2的相关性。结果:DF组外周血SAA、Lp-PLA2、空腹血糖(FBG)、糖化血红蛋白(HbA1c)水平均高于DM组和对照组,差异有统计学意义(P<0.05)。DF狭窄组及闭塞组SAA和Lp-PLA2水平高于下肢动脉斑块组,差异有统计学意义(P<0.05)。Pearson相关性分析,SAA与Lp-PLA2呈正相关(r=0.476,P<0.001)。结论:DF患者血清SAA和Lp-PLA2水平明显升高,重点关注DF患者临床诊断中血清SAA和Lp-PLA2水平的变化,为早期确诊提供依据。

Objective:To analyze the changes and significance of serum amyloid A(SAA)and lipoprotein-associated phospholipase A2(Lp-PLA2)in patients with diabetic foot(DF).Methods:524 cases of patients with type 2 diabetes mellitus(DM)were enrolled,according to whether DF lesions occurred,they were divided into DF group(n=256)and DM group(n=268),another healthy people were selected as the control group(n=270).The changes of serum SAA and Lp-PLA2 levels in the three groups were compared,the serum SAA and Lp-PLA2 levels of DF patients were analyzed according to lower extremity arterial plaque,stenosis and occlusion to explore the correlation between SAA and Lp-PLA2.Results:The levels of peripheral blood SAA,Lp-PLA2,FBG,HbA1c in DF group were higher than those in DM group and control group,the differences were statistically significant(P<0.05).The SAA and Lp-PLA2 levels in DF stenosis group and occlusion group were higher than those in lower extremity arterial plaque group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that SAA and Lp-PLA2 were positively correlated(r=0.476,P<0.001).Conclusion:The serum SAA and Lp-PLA2 levels of DF patients increase significantly,the changes of serum SAA and Lp-PLA2 levels in the clinical diagnosis of diabetic foot patients are emphasized to provide the basis for early diagnosis.
