绷带式角膜接触镜辅助治疗眼化学性烧伤的临床效果 被引量:2

Clinical auxiliary efficacy of bandage contact lens for the treatment of ocular chemical burn

摘要 目的评估绷带式角膜接触镜辅助治疗急性眼化学性烧伤的效果.方法前瞻性病例对照研究.大化集团医院2017年5月至2019年2月急性眼化学性烧伤28例(35只眼),按随机数字表法分为两组:观察组14例(18只眼),对照组14例(17只眼).所有患者进行紧急处理及常规治疗,观察组佩戴绷带式角膜接触镜,对照组不佩戴绷带镜.随访21天,观察视力、角膜愈合情况及并发症.结果治疗后3天观察组视力(BCVA,logMAR)为0.20±0.08,优于对照组的0.31±0.12(t=-3.134,P=0.004).治疗后3、7和14天角膜荧光素染色评分观察组显著低于对照组(Z=-3.304,-3.312,-2.249;P=0.001,0.001,0.024).观察组角膜愈合时间为(9.44±3.93)天,较对照组的(14.82±4.53)天短(t=-3.759,P=0.001).结论佩戴绷带式角膜接触镜可以缩短急性眼化学性烧伤后角膜修复时间,使视力更快恢复. Objective To evaluate the auxiliary efficacy of bandage contact lens for the treatment of acute ocular chemical burn.Methods This was a prospective case control study.Thirty-five eyes of 28 patients with acute ocular chemical burn from May 2017 to Feb.2019 in Dahua Group Hospital were divided into two groups based on random number table method.The observation group included 18 eyes of 14 patients and the control group included 17 eyes of 14 patients.All patients were treated with emergency and conventional treatment.Bandage contact lenses were applied in the observation group in contrary to the control group.The follow-up time was 21 days.The visual acuity,corneal healing time and complications after treatment were observed.Results The visual acuity(BCVA,logMAR)at 3 days after treatment in the observation group was 0.20±0.08,which was better than the 0.31±0.12 in the control group(t=-3.134,P=0.004).The scores of corneal fluorescence staining in the observation group were significantly lower than those in the control group at 3,7 and 14 days after treatment(Z=-3.304,-3.312,-2.249,P=0.001,0.001,0.024).The average time of corneal healing in the observation group was(9.44±3.93)days which was shorter than the(14.82±4.53)days in the control group(t=-3.759,P=0.001).Conclusion Application of bandage contact lens for the treatment of acute ocular chemical burn is effective for shorting the time of corneal healing and recovering the visual acuity.
