胎儿肺动脉吊带的超声心动图表现及诊断思路

Prenatal ultrasound diagnosis of pulmonary artery sling

目的探讨胎儿肺动脉吊带(PAS)的超声心动图特点、诊断思路及诊断价值。方法回顾性分析2011年~2019年我院检出的9例PAS胎儿产前超声图像表现,总结其诊断思路及要点。结果9例PAS胎儿心脏三血管-肺动脉分叉切面均未显示肺动脉分叉,主肺动脉直接延续为左位动脉导管及右肺动脉,于右肺动脉右后侧壁见异常起源的左肺动脉,向左下方走行绕过气管后方进入左肺。9例PAS中,3例合并室间隔缺损;2例合并法洛四联症;3例合并永存左位上腔静脉;2例合并右位主动脉弓;1例合并主动脉缩窄;1例合并颅骨形态异常,呈草莓形。结论产前超声心动图能够较为准确及时的诊断PAS,有利于产前及时咨询和出生后早期临床干预治疗。

Objective To explore the echocardiographic features and value of prenatal diagnosis of pulmonary artery sling(PAS).Methods We retrospectively analyzed the two-dimensional and color Doppler image features of 9 fetuses with PAS from 2011~2019.The diagnostic features of prenatal echocardiography were summarized.Results The main echocardiographic characteristics of PAS of 9 fetus were that the branch of pulmonary artery disappeared,the left pulmonary artery arised from the proximal right pulmonary artery,posterior to the trachea to reaching the left hilum.3 cases associated with ventricular septal defect;2 cases associated with tetralogy of Fallot;3 cases associated with persistent left superior vena cava;2 cases associated with right aortic arch;1 case associated with coarctation of the aorta;and 1 case associated with the strawberry shape of skull.Conclusion PAS can be diagnosed by prenatal echocardiography.Prenatal diagnosis of PAS is helpful prenatal counseling and early treatment.
