照顾者对先天性胫骨假关节手术患儿医疗恐惧认知和以家庭为中心的干预意愿的现况调查

Inventory survey on caregivers'perceptions of medical fear and family-centered intervention willingness in children patients undergoing congenital tibial pseudoarthrosis operation

目的了解照顾者对先天性胫骨假关节手术患儿医疗恐惧认知和干预意愿现状。方法选取2018年1月至2019年12月我科收治的105名先天性胫骨假关节患儿主要照顾者为研究对象,采用照顾者医疗恐惧和干预意愿调查表对其进行问卷调查。结果主要照顾者对患儿的医疗恐惧缺乏深入认识,45.71%不知晓患儿出现医疗恐惧时的表现,仅28.57%知晓如何预防患儿医疗恐惧。主要照顾者对患儿医疗恐惧有着较强干预意愿,且其意愿是可行的,其中,62.86%主要照顾者认为针对患儿行手术预演训练干预是有必要的,71.43%主要照顾者认为手术预演训练有利于改善护理质量与服务,情景游戏是最合适的干预方式,术前1 d是最合适的干预时间,15~30 min是最合适的干预时长,病房护士、家属和患儿共同参与是最合适的形式。结论先天性胫骨假关节手术患儿主要照顾者对手术患儿医疗恐惧缺乏全面深入认知,但其干预意愿强烈,应充分发挥以家庭为中心的干预模式的作用,最终保证患儿安全。

Objective To understand the current situation of caregivers'perceptions of medical fear and intervention willingness in children patients undergoing congenital tibial pseudoarthrosis operation.Methods To select the main caregivers of 105 children patients with congenital tibia pseudoarthrosis who were admitted to our department from January 2018 to December 2019 was set as the study objects,and the caregivers’medical fear and intervention willingness questionnaire were used to conduct a questionnaire survey.Results The main caregivers lacked in-depth understanding of the children patients’medical fears.45.71%of them did not know the behavior of children patients with medical fears,and only 28.57%of caregivers knew how to prevent children patients’medical fears.The main caregivers had a strong willingness to intervene in children patients’medical fears,and their willingness was feasible.Among them,62.86%of the main caregivers believed that it was necessary to conduct surgical rehearsal training interventions for the children patients,and 71.43%of the main caregivers believed that the surgical rehearsal Training was conducive to improving the nursing quality and services.Situational games were the most appropriate intervention method,1 day before surgery was the most appropriate intervention time,15-30 minutes was the most appropriate intervention time,and the participation of nurses,family members and children patients in the ward was the most suitable form.Conclusion The main caregivers of children patients undergoing congenital tibia pseudoarthrosis operation lacked a comprehensive and in-depth understanding of the medical fear of the children patients,but their willingness to intervene was strong.The family-centered intervention model should be fully utilized to ensure the safety of the children patients.
