应用MRI图像分析乳腺肿瘤在仰卧位与俯卧位上的位置变化 被引量:2

摘要 目的通过进行患者俯卧位、仰卧位的磁共振成像(Magnetic Resonance Imaging,MRI),定量评估体位改变时乳腺肿瘤的位置及形态变化。方法本研究以31例乳腺肿瘤患者为研究对象,先进行标准俯卧位MRI扫描,然后进行仰卧位Dixon序列扫描。肿瘤分割后,测量两种体位上肿瘤的体积;以乳头中心点为原点,分析不同体位下肿瘤位置的变化。结果俯卧位和仰卧位肿瘤体积的平均值和标准差分别为(9797.3±9390.6)、(9795.5±9413.1)mm3,两者比较无显著差异(P=0.877)。从俯卧位到仰卧位,以乳头为原点肿瘤平均移动(27.8±3.3)mm。而且,从俯卧位至仰卧位,在冠状位上乳腺外侧区域的肿瘤倾向于沿Z轴向外和向下移动;横轴面中所有区域的肿瘤都趋向于沿Y轴向外、向下移动。结论在俯卧位MRI上位于外侧区的肿瘤,其在仰卧位的位置可以预测。本研究有助于外科医生利用标准俯卧位MR图像定位手术时肿瘤的位置进而指导手术计划制定。 Objective To quantitatively evaluate the location and morphology of breast tumor in prone position and supine position by magnetic resonance imaging(MRI).Methods In this study,31 patients with breast cancer were selected as the research objects.MRI scan in standard prone position was performed first,and then Dixon sequence scanning in supine position was performed.After the tumor was segmented,the volume of the tumor in the two positions was measured,and the changes of the tumor position in different positions were analyzed with the center point of the nipple as the origin.Results The mean and standard deviation tumor volumes of prone and supine were(9797.3±9390.6)mm^3,(9795.5±9413.1)mm^3,respectively,and no significant difference was found between two positions(P=0.877).The average tumor movement at the nipple origin from prone to supine position was(27.8±3.3)mm.Moreover,from prone position to supine position,tumors in lateral breast region tend to move outward and downward along Z axis in coronal view,while tumors in all regions in transverse plane tend to move outward and downward along Y axis.Conclusion In the prone position,the location of the tumors in the lateral region on MRI in the prone position can be predicted.This study is helpful for surgeons to locate the location of tumor during surgery using standard prone position MR images,and then guide the formulation of operation plan.
