数字化乳腺X线摄影与三维断层摄影对乳腺癌初诊价值的比较分析 被引量:8

A Comparative Analysis of Full-field Digital Mammography and Digital Breast Tomosynthesis in the Diagnosis of Breast Cancer

摘要 本文使用全视野数字化乳腺X线成像(Full-Field Digital Mammography,FFDM)、数字化乳腺X线三维断层摄影技术(Digital Breast Tomosynthesis,DBT),以病理诊断结果作为"金标准",取58例(60个病灶)样本进行回顾性研究。研究发现,DBT诊断肿瘤大小测量值与病理诊断比较,差异无统计学意义(P>0.05);而FFDM诊断肿瘤大小测量值显著大于病理诊断结果(P<0.05);DBT在10~20 mm病灶诊断特异性显著高于FFDM诊断(P<0.05);DBT原位癌、非原位癌诊断准确率分别为90.91%、97.96%,总准确率为95.00%,显著高于FFDM诊断(P<0.05)。因此,相对于FFDM,DBT用于乳腺癌诊断,能更加精确地测量病灶大小,且对于10~20 mm径线范围内肿块的诊断特异性更高。DBT对原位癌的诊断未体现出明显优势,但对非原位癌的诊断准确性更高。 With pathological diagnosis results as the"gold standard",the clinical data of 58 cases(60 lesions)samples was analyzed retrospectively by full-field digital mammography(FFDM)and digital breast tomosynthesis(DBT).The study showed that there was no statistically significant difference between the measured value of the tumor size diagnosed by DBT and the pathological diagnosis(P>0.05);while the measured value of the tumor size diagnosed by FFDM was significantly larger than the result of pathological diagnosis(P<0.05);the diagnostic specificity in 10~20 mm lesions by DBT was significantly higher than the diagnosis of FFDM(P<0.05);the diagnostic accuracy of DBT carcinoma in situ and non-in situ carcinoma were 90.91%and 97.96%,respectively,and the total accuracy rate was 95.00%,which was significantly higher than diagnosis of FFDM(P<0.05).Therefore,compared with FFDM,DBT can more accurately measure the size of the lesion,and has a higher specificity for the diagnosis of tumors in the diameter range of 10~20 mm for breast cancer diagnosis.DBT does not show obvious advantages in the diagnosis of carcinoma in situ,but it has higher accuracy in the diagnosis of carcinoma in situ.
