摘要
目的研究早期肠内营养支持对食管癌患者术后应激反应及营养指标的影响。方法选取我院收治的80例食管癌患者,随机分为观察组40例(采用完全肠外营养方案)和对照组40例(肠内营养支持)。观察对比两组患者术前及术后7 d的血清促肾上腺皮质激素(ACTH)和皮质醇(Cor)等应激指标水平,以及血清转铁蛋白(TRF)、血清前白蛋白(PAB)和白蛋白(ALB)等营养指标水平。结果观察组患者血清ACTH、Cor水平较治疗前明显较低(P<0.05),但对照组治疗前后比较差异不明显(P>0.05);观察组患者血清TRF、PAB和ALB水平明显升高(P<0.05),且明显高于对照组(P<0.05)。结论早期肠内营养支持可减轻食管癌患者术后应激反应,改善术后的营养状况,促进术后康复。
Objective To study the effects of early enteral nutrition support on postoperative stress response and nutritional indicators of patients with esophageal cancer. Methods 80 patients with esophageal cancer admitted to our hospital were selected. There were 40 cases in the observation group(using a total parenteral nutrition program) and 40 cases in the control group(enteric nutrition support). The stress indicator levels(serum adrenocorticotropic hormone-ACTH, cortisol-Cor, etc), and nutritional indicators(serum transferrin-TRF, serum prealbumin-PAB and albumin-ALB) were detected and compared in the 2 groups. Results Serum ACTH and Cor levels in the observation group were significantly lower than before treatment(P<0.05), but there were no differences of them in the control group before and after treatment(P>0.05);serum TRF, PAB and ALB levels in the observation group were significantly increased after treatment(P<0.05), and the serum TRF, PAB and ALB levels in the observation group were significantly higher than those in the control group after treatment(P<0.05). Conclusion Early enteral nutrition support can reduce postoperative stress response in patients with esophageal cancer, improve postoperative nutritional status of cancer patients, and promote postoperative recovery.
