摘要
目的:观察胃俞募配穴针刺对胃扩张状态下受试者静息态后扣带回功能连接的影响及其与胃运动变化的相关性,探讨胃俞募配穴针刺效应的后扣带回功能整合机制。方法:24名健康受试者在不同的时间分别纳入中脘组、胃俞组和胃俞配伍中脘组,在水负荷胃扩张状态下分别接受针刺干预,针刺前后对受试者进行静息态脑功能磁共振成像(fMRI)扫描和胃电图检测,分析各组受试者针刺前后静息态后扣带回功能连接网络变化及胃运动变化情况。结果:针刺胃俞募配穴后,受试者后扣带回与双侧脑干、丘脑、额上回、楔前叶,右小脑后叶功能连接增强,后扣带回与双侧前扣带回、枕中回功能连接减弱;与针刺胃俞穴比较,胃俞募配穴针刺引起了受试者后扣带回与双侧楔前叶功能连接增强,与双侧前扣带回、左额上回功能连接减弱;与针刺中脘穴比较,胃俞募配穴针刺引起了受试者后扣带回与双侧丘脑、楔前叶功能连接增强,与双侧颞下回,左额下回、顶下小叶、中央前回,右缘上回功能连接减弱。胃俞募配穴针刺引起的后扣带回-小脑后叶、后扣带回-前扣带回功能连接变化与胃运动变化呈正相关。结论:胃俞募配穴针刺可引起后扣带回与其他脑区功能连接显著改变,后扣带回与楔前叶的功能连接增强与胃俞募配穴针刺的脑功能整合密切相关。
Objective:To observe the posterior cingulate cortex(PCC)functional connectivity(FC)of resting-state brain function in the healthy subjects of gastric distention treated with acupuncture at the back-shu and front-mu points of the stomach[Weishu(BL21)and Zhongwan(CV12)]and the correlation with gastric motility so as to explore the functional integration mechanism of PCC after acupuncturing at the front-mu and back-shu points of the stomach.Methods:A total of 24 healthy subjects recruited by public recruitment,were divided into BL21 group,CV12 group and CV12 plus BL21 group for 3 times at different times,8 in each group every time.A total of 24 subjects were included in each group.All subjects under the water load condition respectively received acupuncture at BL21,CV12 and CV12 plus BL21,and were examined by resting-state functional magnetic resonance imaging(rs-fMRI)and gastric electrogram before and after acupuncture.The FC of PCC and gastric motility were measured before and after acupuncture.Results:After acupuncturing at CV12 plus BL21,the FC between PCC and bilateral brainstem,thalamus,superior frontal gyrus,anterior cuneiform lobe,right posterior cerebellum enhanced,the FC between PCC and bilateral anterior cingulate gyrus,middle occipital gyrus,right precuneus decreased.Compared with BL21 group,acupuncture at CV12 plus BL21 induced the increased FC between PCC and precuneus and the decreased FC between PCC and bilateral anterior cingulate gyrus,left superior frontal gyrus.Compared with CV12 group,acupuncture at CV12 plus BL21 induced the increased FC between PCC and bilateral thalamus,precuneus and the decreased FC between PCC and bilateral inferior temporal gyrus,left inferior frontal gyrus,inferior parietal lobule,central anterior gyrus,right superior marginal gyrus.Conclusion:Acupuncture at front-mu and back-shu points of the stomach can cause significant changes in FC between the PCC and other brain regions.The enhancement of FC between the PCC and precuneus is closely related to the mechanism of brain functional integration of acupuncture at CV12 plus BL21.
