围产期妇女B族溶血性链球菌、支原体感染及耐药情况研究

Study on group B hemolytic Streptococcus and Mycoplasma infection and drug resistance in perinatal women

摘要 目的探讨围产期妇女B族溶血性链球菌(GBS)和支原体的感染及耐药情况,为临床预防及抗菌药物的合理使用提供依据。方法2016年10月-2018年12月在郑州市中心医院定期做围产保健孕妇共4454例,均采取阴道及肛门直肠拭子分别进行GBS和支原体检测。结果GBS阳性率为9.9%,支原体阳性率为17.3%;随年龄增长支原体的感染呈上升趋势,B群感染与年龄无明显相关性;B群对氨苄西林、头孢噻肟素、青霉素、万古霉素、头孢曲松和利奈唑胺的敏感性较高,解尿脲原体对美满霉素、强力霉素、罗红霉素的耐药率较低。人型支原体对美满霉素、强力霉素、交沙霉素、加替沙星的耐药率较低。结论加强围产期孕产妇产前GBS及支原体筛查,在孕期给予重点的健康保护和健康教育的同时也要加强孕期筛查工作,必要时在产前以及产时进行重复检测,早发现、早治疗,避免其对分娩结局造成不良影响。 Objective The purpose of this study is to investigate the infection and drug resistance of group B hemolytic Streptococcus(GBS)and Mycoplasma in perinatal women,so as to provide basis for clinical prevention and rational use of antibacterial drugs.Methods A total of 4454 pregnant women with perinatal care in Zhengzhou Central Hospital from October 2016 to December 2018 were collected for analysis.The vaginal and anal rectal swabs were used for GBS and mycoplasma detection.Results The positive rate of GBS was 9.9%,the positive rate of Mycoplasma was 17.3%;Mycoplasma infection increased with age,but group B hemolytic Streptococcus infection had no significant correlation with age.The results of drug resistance showed that group B hemolytic Streptococcus was highly sensitive to ampicillin,cefotaxime,penicillin,vancomycin,ceftriaxone and linezolid.Conclusion Prenatal GBS and Mycoplasma screening of perinatal pregnant women should be strengthened.In addition to key health protection and health education during pregnancy,screening during pregnancy should also be strengthened.If necessary,repeated tests should be carried out before and during childbirth to detect and treat early so as to avoid adverse effects on the outcome of childbirth.
