摘要
目的探讨Th22细胞和Th17细胞及其细胞因子水平在溃疡性结肠炎(UC)患者中的变化及意义。方法选取UC患者156例,其中缓解期64例,活动期92例,并根据改良Mayo评分将活动期UC分为轻度组34例、中度组30例和重度组28例,选取健康体检者60例为对照组,对所有入选者进行Th17细胞和Th22细胞数量及血浆IL-22和IL-17A的浓度等测定。结果活动组和缓解组Th22细胞比例、Th17细胞比例、血浆IL-22和IL-17A水平均高于对照组,并且活动组Th22细胞比例、Th17细胞比例、血浆IL-22和IL-17A水平均高于缓解组,差异均有统计学意义(P<0.05);在UC活动期患者中,重度组Th22细胞比例、Th17细胞比例、血浆IL-22和IL-17A水平均高于轻度组和中度组(P<0.05);在UC患者中,Th22细胞和Th17细胞比例与ESR、CRP和Mayo评分均呈正相关。结论UC患者中Th22细胞和Th17细胞比例明显升高,且与疾病活动度及炎症程度密切相关。
Objective To explore the role of T helper(Th)22 and Th17 cells and their cytokines in the pathogenesis of ulcerative colitis(UC).Methods A total of 156 patients with UC were enrolled,including 92 cases of active UC patients and 64 cases of inactive UC patients,and 60 healthy individuals were selected as the normal controls.Patients with active UC were divided into subgroups according to the modified Mayo scoring:34 cases in the mild group,30 cases in the moderate group,and28 cases in the severe group.Their frequencies of Th22 and Th17 cells in peripheral whole blood and interleukin(IL)-22 and IL-17 A in plasma were determined.Results Active UC patients and inactive UC patients had higher percentage of Th22 cells,the percentage of Th17 cells,plasma IL-22 level and plasma IL-17 A level than the control group(P<0.05).Active UC patients had higher percentage of Th22 cells,percentage of Th17 cells,plasma IL-22 level and plasma IL-17 A level than inactive UC patients(P<0.05).Patients in the severe group had higher percentage of Th22 cells,percentage of Th17 cells,plasma IL-22 level and plasma IL-17 A level than patients in the mild and the moderate groups(P<0.05).In patients with UC,the percentage of Th22 cells and the percentage of Th17 cells were both positively correlated with ESR,CRP and modified Mayo scoring.Conclusion The percentage of Th22 cells and the percentage of Th17 cells are higher in patients with UC,which are correlated with the degree of disease activity and inflammation.
