实时剪切波弹性成像在慢性肾脏病的诊断及评估分期中的应用价值 被引量:2

摘要 目的:探讨运用实时剪切波弹性成像在慢性肾脏病(CKD)的诊断及评估分期中的临床应用价值。方法:选择我院2017-2018年收治的100例慢性肾脏病患者为研究组,并按病情分为5组(CKD1-5期组),以同期100例健康体检者作为对照组,运用实时剪切波弹性成像检测受检者肾脏的杨氏模量值,对比分析慢性肾脏病患者与健康体检者实时剪切波弹性成像检测结果的差异性,并分析慢性肾脏病各期患者的实时剪切波弹性成像检测结果的差异性。结果:慢性肾脏病组左肾中下部肾皮质、肾髓质、肾窦部杨氏模量值均高于健康体检组,差异有统计学意义(P<0.05)。慢性肾脏病1期与2期患者左肾中下部肾皮质的杨氏模量值比较无明显差异(p>0.05),慢性肾脏病3期、4期、5期患者左肾中下部肾皮质的杨氏模量值比较差异显著(p<0.05)。结论:实时剪切波弹性成像可无创定量评价慢性肾脏病患者的纤维化程度,在慢性肾脏病的诊断及评估分期中,值得临床推广应用。 Objective:To evaluate the clinical applicationvalueof real-time shear wave elastography in the diagnosis and staging of chronic kidney diease.Methods:100 patients with chronic kidney disease treated in our hospital from 2017 to 2018 were selected as the study group,and divided into CKD1-5 stage group.100 cases of healthy physical examination in the same period were selected as the control group.Using real-time shear wave elastography to detection the Young’s modulus of kidney.The differences of detection results of real-time shear wave elastography were compared between CKD group and control group.Results:Compared with the control group,the value of Young’s modulu in the left kidney cortex,medulla,and sinus in CKD group was higher,and the difference is statistically significant(P<0.05).The value of Young’s modulu was no significant difference in CKD1 stage group and CKD2 stage group(P>0.05).The value of Young’s modulu was significant difference in CKD1 stage group,CKD2 stage group and CKD3 stage group(P<0.05).Conclusion:Thereal-time shear wave elastography can be used to evaluate the degree of fibrosis in patients with CKD noninvasively and quantitatively.It is worth popularizing and applying in the diagnosis and evaluation stage of CKD.
