基于酸甘化阴理论乌梅喷雾剂改善维持性血液透析患者口渴症状及生活质量的研究

The dark plum aerosol improve the thirsty degree and quality of life in uremic patients with hemodialysis based on Theory of Sour-sweet Herbs Nourishing Yin

目的:基于酸甘化阴理论,探讨中药乌梅喷雾剂对尿毒症血液透析患者口渴症状、透析相关指标及生活质量的影响。方法:2017年9月至2018年2月,选取血液透析室筛选为口渴症的维持性血液透析患者60例,采用信封法随机分为对照组和观察组,每组30例。在常规护理的基础上,分别给予对照组温开水喷雾,观察组乌梅喷雾剂喷雾,收集并比较两组患者口渴程度、静息唾液流率、平均脱水量及肾脏疾病相关生活质量。结果:干预前两组患者基线资料及口渴强度无显著性差异(P>0.05)。喷雾后,口渴程度评分显示观察组为(21.8±1.5)分、对照组为(28.7±1.8)分,两组差异具有统计学意义(P<0.01)。2min唾液静息流率观察组和对照组分别为(0.97±0.484)2min/mL和(0.31±0.134)2min/mL,差异具有统计学意义。喷雾前后,平均超滤量观察组分别是(2855.10±750.01)及(2759.59±750.73)呈下降趋势,而对照组分别是(2987.55±861.81)及(3022.79±823.96)则有增加趋势。干预后KDQOL-SFTM 1.3评分中SF-36部分观察组生理功能、心理健康、情感职能、社会功能、健康状况有明显的改善(P<0.01),KDTA部分,在透析相关症状、肾脏病影响、工作状态、睡眠等患者客观感受等项目上,观察组的评分高于对照组,组间比较差异有统计学意义(均P<0.05)。结论:尿毒症患者可通过使用乌梅喷雾降低其口干口渴程度,从而提高生活舒适。同时通过控水降低其体质量增加量而减少透析脱水量,心血管的负担减轻可减少尿毒症患者透析并发症的发生,具有一定的临床护理意义。

Objective:To study clinical effect of dark plum aerosol on the dialysis index,thirsty degree and Quality of Life in uremic patients with hemodialysis.Methods:All of 60 hemodialysis patients with the syptom of thirsty were randomly divided into the observation group(n=30)and the control group(n=30).The observation group were given dark plum aerosol based on the routine treatment.the control group were given warm water spray based on the routine treatment.before and after intervention,the degree of thirsty,resting salivary flow rates,ultrafiltration and the quality of life were evaluated.Results:Before the intervention,The difference of baseline data and the thirsty degree between two groups was not statistically significance(P>0.05).After the therapy,the thirsty degree of observation group(21.8±1.5)decreased obviously when compared with control group(28.7±1.8)(P<0.05).The 2 min resting salivary flow rates of the experimental group and the control group were respectively(0.96±0.484)min/mL and(0.31±0.134)min/mL and it had a statistically significant difference(P<0.01).The average magnitude of the hemodialysis ultrafiltration of observation group was on the decline while the control group's was uptrend.In the investigation of KDQOL-SFTM 1.3,SF-36 shows that the physiological function,mental health,emotional function,social funtion and health status has improved in the observation group(P<0.01).KDTA shows that hemodialysis correlative symptoms,effects of kidney disease,work status and sleep in the observation group was than that in the contral group(P<0.05).Conclusion:Dark Plum Aerosol can availably alleviate the degree of thirsty in uremic patients with hemodialysis.This relieves the patients’cardiovascular burden to Improve quality of Life,The influence of dark plum aerosol on the thirsty degree in uremic patients with hemodialysis is valuable to apply for the clinical.
